InBody Scan Procedure — Description and Instructions

The InBody bioelectrical impedance analysis (BIA) scan is a non-invasive body composition assessment that estimates lean mass, fat mass, body water, and related metrics by passing a low-level electrical signal through the body. The scan does not involve radiation. Results are intended to support clinical assessment and wellness planning and are not a substitute for medical diagnosis.

I have fasted for at least 2 hours prior to the scan.
I have abstained from alcohol for 24 hours prior to the scan.
I have avoided heavy exercise for 12 hours prior to the scan.
I will remove all jewelry and metal accessories prior to the scan.

Risks, Limitations, and Contraindications

By signing below I acknowledge that I have been informed of the following: InBody scanning is generally safe for most individuals. The device uses imperceptible electrical currents; however, the procedure is contraindicated for individuals with implanted electronic medical devices such as pacemakers, implantable cardioverter-defibrillators (ICDs), or neurostimulators. If you have such a device, you must not undergo the scan. Accuracy of results may be affected by recent food or fluid intake, extreme hydration or dehydration, presence of metallic implants, or aberrant physiology. The scan is not diagnostic of disease. Minor skin irritation from contact with electrodes is possible but uncommon.

I understand that I may refuse or withdraw consent at any time prior to the scan without penalty and that withdrawing consent will not affect my access to other services.

Authorization to Perform Scan and Release of Results

I hereby authorize clinical staff to perform the InBody scan and to collect and store the resulting measurements in my health record. I authorize release of the InBody results to the clinicians and authorized staff involved in my care. I understand results may be used for treatment planning, clinical tracking, and quality improvement.

Patient Certification

I certify that the information I have provided on this form is true and complete to the best of my knowledge. I have read and understand the description of the InBody scan, the stated risks and limitations, and the contraindications. I have had an opportunity to ask questions and have received satisfactory answers. I consent to the performance of the InBody scan and the release and storage of results as described above.

What the Healthcare InBody Scan Form Is and when it’s used

The Healthcare InBody Scan Form documents patient consent, basic demographics, and measurement results from an InBody body composition device used in clinical or wellness settings. It records the date, operator, measured variables (weight, body fat percentage, skeletal muscle mass, water distribution), and any clinician notes. The form supports clinical tracking, billing codes where applicable, and patient records. When combined with a signed authorization, it becomes part of the patient’s health record and may be subject to HIPAA and facility retention policies.

Why a standardized InBody Scan Form matters for care and compliance

A consistent Healthcare InBody Scan Form ensures accurate tracking of body composition over time, documents patient consent, and creates an auditable record for clinical decisions. Properly completed forms reduce data entry errors and help clinics meet HIPAA documentation and retention expectations.

Why a standardized InBody Scan Form matters for care and compliance

Who typically completes and signs this form

Typical users include clinical staff administering the scan and the patient providing consent; administrators may also review and store the record.

  • Clinical technician or nurse completes measurement fields and confirms device calibration before scanning.
  • Patient reviews results, signs consent for the scan, and acknowledges data use and sharing terms.
  • Health records or administrative staff enter the scanned data into the electronic medical record and archive the signed form.

Assign clear role responsibilities at your clinic to ensure consistent completion, signature capture, and secure storage of each Healthcare InBody Scan Form.

Essential sections to include on a professional form

A complete Healthcare InBody Scan Form groups patient identifiers, measurement output, consent language, operator details, and signature blocks to support clinical use and legal compliance.

Patient Info

Full legal name, date of birth, medical record number, and contact details. Accurate identifiers link the scan to the correct health record and prevent misattribution across patients.

Measurement Data

All InBody outputs (weight, body fat %, skeletal muscle mass, visceral fat rating, total body water) recorded with units and device serial number for auditability and future comparison.

Clinical Notes

Space for clinician observations, hydration status, recent activity or food intake that may affect readings, and recommended follow-up to contextualize the measurements.

Consent Statement

Clear patient consent language describing purpose of the scan, how results will be used, and any data-sharing specifics required under HIPAA for disclosures or research use.

Operator Details

Name, role, facility department, device calibration status, and date/time of scan to support traceability and device maintenance records.

Signature Block

Signature and printed name of patient and operator plus date/time. Include space for guardian signature when the patient is a minor or legally incapable.

Common mistakes to avoid when preparing the form

  • Using nicknames or initials instead of the patient’s full legal name causes record mismatches and billing delays.
  • Forgetting to record device serial or calibration status makes the measurement non-reproducible for quality audits.
  • Accepting unsigned or initialed consent where a full signature is required undermines legal defensibility of the record.
  • Entering results without noting relevant contextual data (recent exercise, fluid intake) can mislead clinical interpretation.

Risks and compliance consequences of incorrect forms

HIPAA Violation: Potential fines and corrective action
Misdiagnosis Risk: Clinical decisions may be compromised
Billing Denials: Claims may be rejected
Malpractice Exposure: Increased liability risk
Data Breach Risk: Regulatory penalties possible
Record Loss: Disrupted continuity of care
