Intern will have access to confidential and protected health information (PHI). Intern shall not access, use, or disclose PHI except as permitted and required to perform assigned duties. Intern shall comply with all applicable federal and state privacy laws and Facility privacy policies. Unauthorized use or disclosure of PHI is grounds for disciplinary action, including termination and potential civil or criminal liability.

BACKGROUND CHECKS, IMMUNIZATIONS, AND HEALTH REQUIREMENTS

As a condition of placement, Intern affirms and warrants that all required pre-placement screening, background checks, immunizations, and health examinations have been completed in accordance with Facility policy and applicable law. Intern attests to the following (check applicable):




INSURANCE, LIABILITY, AND COMPENSATION

Intern understands that Facility does not guarantee employment and that the internship may be unpaid or compensated as follows:

Facility maintains professional liability insurance covering its employees and agents. Intern is responsible for maintaining their own health insurance coverage and personal liability insurance as required by their educational institution unless otherwise agreed in writing.

TERMINATION

Either party may terminate this Agreement at any time, with or without cause, by providing written notice to the other party. Termination for cause may be effective immediately where patient safety, breach of confidentiality, failure to comply with Facility policy, or criminal conduct is involved.

INDEMNIFICATION

To the extent permitted by law, each party shall defend, indemnify, and hold harmless the other party from claims, liabilities, damages, and expenses arising from the indemnifying party's negligence or willful misconduct in connection with this Agreement.

GOVERNING LAW; VENUE

This Agreement shall be governed by the laws of the state where Facility is located. Venue for any dispute arising under this Agreement shall lie exclusively in the courts of that state.

ENTIRE AGREEMENT; AMENDMENT
