Treatment Description

Invisalign clear aligner therapy is an orthodontic system using a series of removable, custom-fitted thermoplastic aligners to move teeth incrementally. Treatment may include attachments bonded to teeth, interproximal reduction (IPR) to reshape tooth surfaces, and the use of elastics or auxiliaries when indicated. The estimated duration of active aligner therapy is months, subject to clinical change.

Select any procedures that may be performed as part of the treatment plan:

Bonding of composite attachments to teeth for aligner retention
Interproximal reduction (IPR) / enamel reshaping
Use of elastics or auxiliaries
Post-treatment refinements (additional aligners) may be required

Risks, Potential Complications and Acknowledgements

I acknowledge that my practitioner has explained the nature of Invisalign treatment, its expected benefits, reasonable alternatives (including no treatment, braces, or other appliances), and the material risks and potential complications associated with treatment, including but not limited to the following. I understand that not all risks can be listed.

Tooth and/or root sensitivity, transient or chronic, including increased sensitivity to cold, heat, biting or percussion.
Soft tissue irritation, ulceration or soreness from aligners, attachments, or elastics.
Root resorption (shortening of tooth roots) which may be permanent and could affect tooth stability.
Possible need for endodontic treatment (root canal) or extraction in rare cases.
Temporomandibular joint (TMJ) discomfort, pain, or dysfunction may occur or be aggravated.
Relapse or movement after treatment if retainers are not worn as instructed; long-term retention typically required.
Unpredictable tooth movements or incomplete correction requiring additional treatment or refinements.
Allergic or adverse reactions to aligner materials, bonding agents or metals used in adjunctive appliances.

I understand that successful treatment requires compliance with wearing aligners as instructed, attending scheduled appointments, and following home care instructions. Failure to comply may prolong treatment time, increase costs, and reduce the likelihood of achieving the planned outcome.

HIPAA/Privacy & Authorization

I authorize the release and disclosure of my dental and medical records, including diagnostic records, dental models, photographs, radiographs and treatment notes, to the treating clinicians, the dental laboratory, and other healthcare professionals as necessary to provide and coordinate my care. I understand that this authorization is voluntary and that I may revoke it in writing, except to the extent that action has already been taken pursuant to this authorization.

I acknowledge receipt of the practice privacy policy and authorize release of information as described above.

Financial Responsibility & Other Terms

I acknowledge that I am financially responsible for all fees associated with the proposed treatment program regardless of insurance coverage. I understand that additional charges may apply for attachments, IPR, refinements, missed appointments, or restorative work necessary to achieve treatment goals. The clinician has not guaranteed a cure or complete correction of orthodontic problems.

I have had the opportunity to ask questions and have received satisfactory answers about the proposed treatment, alternatives, anticipated results, risks and potential complications. I understand that I may withdraw consent at any time prior to or during active treatment by notifying the treating clinician, although withdrawal may not relieve me of financial responsibility for services already rendered.

Minor Consent (if applicable)

Patient is a minor and the following parent/guardian is providing consent:

What the Healthcare Invisalign Consent Form Is

The Healthcare Invisalign Consent Form documents a patient’s informed agreement to begin Invisalign orthodontic treatment. It typically records the patient’s legal name and contact information, medical and dental history, the prescribed aligner treatment plan, expected timeline, risks, benefits, alternatives, and financial responsibility. The form also documents data-sharing permissions and HIPAA-related disclosures necessary for clinical records. When signed correctly, it becomes an enforceable record under U.S. electronic signature law (see 15 U.S.C. §7001 and applicable state UETA statutes), provided intent, consent, attribution, and retention requirements are satisfied.

Why a Clear, Compliant Consent Form Matters

A complete Healthcare Invisalign Consent Form reduces clinical risk, ensures patients understand treatment tradeoffs, and creates a documented basis for shared decision-making. Properly executed consent also supports regulatory compliance with HIPAA privacy rules and preserves legal defensibility under the ESIGN Act and state UETA frameworks.

Why a Clear, Compliant Consent Form Matters

Who Typically Completes and Signs This Form

Multiple clinic roles are involved in preparing and obtaining consent; responsibilities vary by practice size and workflow.

  • Orthodontists and Dentists responsible for explaining the Invisalign plan and recording clinician statements about expected outcomes and risks.
  • Clinic Administrative Staff prepare templates, manage patient records, and initiate electronic signature workflows or in‑office signing sessions.
  • Patients and Legal Guardians provide acknowledgement of risks, consent to treatment, and signature authority for minors or persons under guardianship.

Understanding who completes, reviews, and stores the form helps allocate tasks and reduces errors during patient intake and treatment planning.

Essential Security and Compliance Elements

Encryption: TLS 1.2/1.3 in transit; AES‑256 at rest
HIPAA BAA: BAA required for cloud storage and eSignature vendors
Audit Trail: Timestamped actions and signer attribution
Access Controls: Role-based access and least-privilege settings
Authentication: Email, SMS code, or advanced signer verification
Retention Policy: Record retention and tamper-evident storage

Consequences of Incomplete or Incorrect Consent

Clinical Liability: Increased malpractice exposure
Regulatory Fines: HIPAA penalties for privacy failures
Invalid Consent: Consent may be legally challenged
Treatment Delays: Procedure postponed for missing signatures
Billing Disputes: Insurance or patient payment issues
Data Breach Risk: Patient information exposure

Core Components of a Professional Consent Form

A well-constructed Healthcare Invisalign Consent Form combines clear clinical details, informed-consent language, and administrative metadata to support care delivery and compliance obligations.

Patient Identity

Full legal name, date of birth, and a government‑issued ID reference to match the clinical record and avoid mismatched records during treatment.

Treatment Plan

Detailed Invisalign plan description including objectives, estimated number of aligners, expected duration, and clinician-specified milestones or refinement scenarios.

Risks and Benefits

Concise explanation of common and material risks, likely benefits, and reasonable expectations so the patient can make an informed decision.

Alternatives

Description of viable treatment alternatives (e.g., traditional braces, no treatment) and why Invisalign was recommended for the patient.

Financial Terms

Clear statement of costs, payment responsibility, refund or cancellation policies, and insurance processing expectations to prevent billing disputes.

Consent & Signatures

Signature block for patient or guardian, date, witness or notary field if required, and a record of how consent was obtained (in person, remote eSign, or RON).

Step-by-Step: How to Complete and Obtain Consent

Follow these sequential steps to gather a valid, auditable consent for Invisalign treatment.

  • 01
Prepare Form: Populate patient and treatment details before presenting to the signer.
  • 02
Review With Patient: Explain risks, benefits, and alternatives verbally and document the discussion.
  • 03
Obtain Signature: Collect patient or guardian signature and date; use eSign or in‑person signing.
  • 04
Store Record: Save the signed document with the treatment chart and audit trail.

How to Configure an Online Consent Workflow

Standardize electronic workflows to ensure consistent capture, authentication, and storage of consent forms.

Field Configuration
Authentication Email link or SMS code; use stronger KBA for higher-risk cases
Template Lock clinical fields and allow only patient-editable sections
Conditional Fields Reveal guardian signature fields when patient is a minor
Storage Save signed PDF to EHR and secure cloud archive

Where to Send or File the Completed Form

Routing should ensure the signed consent is accessible to clinical staff and retained in the official medical record.

  • Clinic EHR: Attach signed PDF to the patient’s electronic health record
  • Patient Copy: Provide patient a downloadable signed copy for their records
  • Billing Team: Send financial consent details to billing or insurance processors
  • Secure Archive: Store a tamper-evident copy in long-term secure storage

Technical Considerations for eSigning and eSubmission

Choose a platform that supports HIPAA compliance, strong audit trails, and the file formats you use in the clinic.

  • Integrations: Connectors to EHR or cloud storage
  • File Formats: PDF and DOCX supported
  • Authentication: SMS, email, or advanced methods

Ensure the vendor provides a BAA for protected health information, supports secure export to your EHR, and preserves an audit trail for regulatory review.

Timing and Key Deadlines to Observe

Track time-sensitive steps so treatment is not delayed and records remain compliant.

Consent Before Treatment:

Obtain signed consent prior to first aligner delivery whenever practicable

Minor Consent Rule:

Parental or guardian signature required before treatment for minors

Emergency Exception:

Document retrospective consent when emergency treatment prevents prior signing

Plan Changes:
