Recitals and Purpose

The Provider will deliver a Treatment Plan as described below. The Treatment Plan may include medication management, psychotherapy, community supports, monitoring, and other clinically appropriate interventions.

Obligations of Patient

The Patient agrees to: attend scheduled appointments, take prescribed medications as directed, participate in treatment activities, notify the Provider of significant changes in condition, and comply with agreed monitoring procedures. Failure to meet these obligations may result in modification of services or notification to supervising authorities where required by law.

Consent for medication administration: I consent to the Provider prescribing and managing psychotropic medications as clinically indicated.

Consent for random drug/alcohol screening (if applicable): I consent to periodic screening as part of monitoring adherence to treatment.

Obligations of Provider

The Provider agrees to: deliver clinically appropriate services in a timely manner; document the Treatment Plan and progress; coordinate care with other authorized providers and supports; and inform the Patient of material changes to treatment or anticipated risks.

Risks, Benefits, and Alternatives

The Patient acknowledges that benefits of treatment may include symptom reduction and improved functioning. Risks may include medication side effects, emotional discomfort during therapy, and potential for limited effectiveness. Reasonable alternatives, including inpatient treatment, refusal of specific medications, or escalation to other community resources, have been discussed as appropriate.

Noncompliance, Review, and Remedies

Noncompliance with material terms may prompt a documented review, intensified case management, referral to higher level of care, or notification to a court or supervising authority where required by law. The Provider will endeavor to use least-restrictive measures and will document rationale for any escalated actions.

Confidentiality and HIPAA Authorization

The Provider will maintain confidentiality in accordance with applicable health information privacy laws. By signing this Contract, the Patient authorizes the Provider to exchange treatment-related information with the following persons or agencies as necessary to implement the Treatment Plan and to comply with legal obligations:

HIPAA Acknowledgment: I acknowledge that I have received and reviewed the Provider's privacy notice and authorize disclosures as necessary for treatment, payment, and healthcare operations, subject to the limits described above.

Revocation and Right to Withdraw

The Patient may withdraw consent to voluntary components of this Contract at any time by providing written notice, except to the extent that rights or obligations are restricted by a court order or other applicable law. Withdrawal will not affect disclosures already made in reliance upon this Contract.

Dispute Resolution & Governing Law

Parties will attempt to resolve disputes regarding clinical or administrative matters through informal discussion and clinical review. This Contract shall be interpreted in accordance with applicable law. Nothing in this provision prohibits either party from pursuing emergency remedies permitted by law.
