By signing below I authorize approved maintenance personnel to enter the premises and to perform the maintenance activities specified above. I understand that only the minimum necessary protected health information will be disclosed to maintenance personnel to permit safe and effective performance of services. Disclosed information may include patient name, address, relevant medical conditions, equipment type, and safety considerations.

I acknowledge that I am responsible for removing items that would impede safe access to equipment and for informing technicians of known hazards. I agree to notify the provider in writing to revoke this authorization; however, revocation will not affect actions already taken in reliance upon this authorization. This authorization expires on the date indicated below or upon completion of the requested maintenance, whichever occurs first.

The organization will exercise reasonable care in performing services but is not liable for pre-existing conditions, existing structural defects, or unforeseeable damage not caused by negligent performance of maintenance. Where local law permits, the organization’s liability for direct damages arising from maintenance is limited to the cost of the service performed. The patient or signer agrees to indemnify and hold harmless the organization and its contractors from claims arising from the patient’s failure to disclose material information or to prepare the premises for safe service.
