By signing this Agreement, Patient authorizes Provider to submit claims to the Medicaid program, to receive Medicaid benefits on behalf of the Patient, and to release medical and other information necessary to process claims. Patient assigns and irrevocably transfers to Provider all rights to present claims to Medicaid, to receive payment for covered services, and to pursue administrative and judicial remedies to recover payment for services provided under this Agreement.

Patient acknowledges that Medicaid may deny payment for services that are not covered or that are deemed nonmedical or elective. Patient agrees to be financially responsible for copayments, deductibles, non-covered items, and services not authorized or paid by Medicaid in accordance with applicable law. Patient further authorizes Provider to obtain third-party liability information and to pursue third-party payers when permitted.

Patient authorizes the release of any medical or other information necessary for Medicaid eligibility determination, utilization review, quality assurance, billing and payment of claims. This authorization includes disclosure to Medicaid contractors, managed care entities, and entities conducting audits or utilization reviews.
