What the Healthcare Medical Release and Fee Contract Is

The Healthcare Medical Release and Fee Contract is a combined authorization and fee agreement used to document a patient's consent to release medical information, permit data sharing with specified parties, and confirm payment terms for services. It typically names the patient and recipient(s), specifies the scope and duration of the release, records any fees or billing arrangements, and includes signature blocks for the patient and an authorized provider representative. Proper completion is essential for HIPAA compliance, billing accuracy, and establishing the legal basis for sharing protected health information.
