I acknowledge that the proposed procedure or treatment and ancillary services have been explained to me, including the expected benefits, material risks, potential complications, and reasonable alternatives. I understand that not all risks can be listed and that complications may include, but are not limited to, infection, bleeding, allergic reaction, scarring, nerve injury, and in rare cases, serious disability or death.
