I authorize the Provider named above to furnish such medical care, services, and supplies as are considered necessary by the Provider for the treatment described above. I understand that reasonable alternatives, material risks, and expected benefits have been explained to me where applicable.

Assignment of Benefits and Billing Authorization

By signing this agreement I authorize the Provider to submit claims to the Medicare program (Title XVIII of the Social Security Act) and to any supplemental or secondary insurer on my behalf. I assign to the Provider all payments for medical benefits to which I am entitled under Medicare and any other health insurance plan for services furnished by the Provider. I authorize any holder of medical or other information about me to release to Medicare, insurers, or its agents any information needed to determine benefits or the benefits payable for related services.

I acknowledge that Medicare may pay less than the actual charges for services and that I am responsible for any applicable co-payments, deductibles, co-insurance, non-covered services, and charges for services denied by Medicare or a secondary payer. The Provider may bill me directly for such amounts.

Privacy, Authorization and Release

I acknowledge receipt of the Provider's Notice of Privacy Practices. I authorize the use and disclosure of my protected health information for treatment, payment, and health care operations, including disclosure to Medicare, insurers, and their agents as necessary to process claims and coordinate benefits. This authorization includes disclosure of records relating to communicable disease, mental health treatment, and substance use treatment to the extent necessary for payment and care coordination.

Acknowledgments and Certifications

I certify under penalty of law that the information I have provided in this Agreement is true, accurate, and complete to the best of my knowledge. I understand that knowingly providing false information or making false statements for the purpose of obtaining payment from Medicare or other insurers may subject me to civil or criminal penalties.

Authorization Period and Revocation

This authorization to bill and to release information is effective upon my signature below and will remain effective until: Expiration Date: unless earlier revoked in writing. Any revocation will not apply to information already released or claims already submitted in reliance on this authorization.
