HEALTHCARE MENTAL HEALTH BILLING CONTRACT

This Mental Health Billing Contract (Contract) is entered into by and between the Provider and the Client identified below. The parties agree that Provider will render billing services and, where applicable, submit claims to third-party payors for mental health services delivered to Client, subject to the terms and conditions set forth in this Contract.

Billing Terms and Payment Responsibilities

Client agrees to pay all copayments, coinsurance, deductible amounts and any portion of fees not paid by insurance within 30 days of billing. If Client elects to self-pay, Client will be responsible for the full session fee at time of service unless other arrangements are made in writing.

Assignment of Benefits and Authorization: Client authorizes Provider to submit claims, to receive payment directly from third-party payors on behalf of Client, and to release medical information necessary to process claims and appeals. Client assigns to Provider all benefits payable for services rendered under this Contract to the extent of Provider's charges. Client remains personally responsible for any balance not paid by insurance.

Client agrees to provide at least 24 hours' notice for cancellation. Late cancellations or missed appointments may be charged a fee of $ . Repeated late cancellations or no-shows may result in termination of services.

Claims, Appeals, and Collections

Provider will make reasonable efforts to submit accurate claims and to pursue reasonable appeals on Client's behalf for reimbursement. Provider is not obligated to pursue appeals beyond initial insurer review unless otherwise agreed in writing. If insurance payments are denied and Client requests Provider to pursue further appeals, additional fees for administrative time may apply and will be billed to Client.

If Client account is referred to an outside collection agency or attorney for collection, Client agrees to pay reasonable collection costs, attorneys' fees and court costs incurred by Provider.

Privacy, HIPAA and Authorization to Release Information

Client acknowledges receipt of Provider's Notice of Privacy Practices and authorizes Provider to use and disclose protected health information (PHI) as necessary for billing, treatment, payment and healthcare operations, including preparation and submission of claims, communications with payors and appeals. Client understands that disclosures for payment purposes may include diagnosis, dates of service, and treatment codes.

Representations; Warranties; Client Obligations

Client represents that the information provided herein is true, accurate and complete. Client will notify Provider of any changes to insurance coverage, address or other contact information within 10 days of the change. Client acknowledges that failure to provide accurate insurance information may result in direct billing to Client.

Provider does not guarantee insurance coverage or payment. Estimates provided are not guarantees. Client remains responsible for fees not covered by insurance.

Termination; Amendment; Governing Law

Either party may terminate this Contract upon 30 days' written notice. Termination does not affect Client's obligation to pay outstanding charges for services provided prior to termination. This Contract may be amended only in a writing signed by both parties. This Contract is governed by the laws of the state in which Provider principally practices.

Severability: If any provision of this Contract is held invalid or unenforceable, the remaining provisions will remain in full force and effect.
