PERMITTED AND PROHIBITED ACTIVITIES

The mentee is permitted to perform only those clinical procedures for which specific authorization and supervision are documented in this Addendum. The mentor will document permitted procedures and any limitations in the mentee's training record. The mentee is expressly prohibited from acting as an independent practitioner, ordering independent care, or representing themselves as credentialed for activities beyond this Addendum.

HEALTH, TRAINING, AND COMPLIANCE

The mentee certifies that the information below is accurate and acknowledges that compliance with facility policies and applicable law is a condition of participation in the mentorship program.

CONFIDENTIALITY AND PATIENT INFORMATION

Unauthorized use or disclosure of protected health information is grounds for immediate termination and may result in civil or criminal penalties.

REPORTING, INCIDENTS, AND TERMINATION

The mentee must promptly report any adverse event, patient safety concern, or breach of confidentiality to the mentor and the facility's designated reporting authority. Either party may terminate this Addendum for cause upon written notice setting forth the grounds for termination. Where feasible, a ten (10) calendar day cure period will be provided for non-safety related breaches.

Grounds for immediate termination include, but are not limited to: willful breach of patient confidentiality, clinical practice beyond authorized scope, conviction of a disqualifying offense, falsification of training or immunization records, or failure to maintain required licensure or insurance.

LIABILITY, INDEMNITY, AND GOVERNING LAW

Each party retains responsibility for their professional acts. The mentee agrees to indemnify and hold harmless the mentor and the mentor's employer from claims arising from the mentee's breach of this Addendum or negligent acts performed outside the authorized scope. This Addendum shall be governed by the laws of the state specified below.

EVALUATION AND DOCUMENTATION

The mentor will maintain a written record of mentoring sessions, competencies assessed, and any corrective actions. Periodic formal evaluations will be conducted at intervals agreed by the parties and documented in the mentee's training file.

MISCELLANEOUS

This Addendum constitutes the entire agreement between the parties with respect to mentorship terms described herein and supersedes prior oral or written agreements to the extent they conflict. Modifications must be in writing and signed by both parties. If any provision is held invalid, the remainder will continue in full force.
