I, the undersigned, hereby consent to participate in an opioid use disorder treatment program that includes the administration of methadone. I acknowledge that I have been given the opportunity to ask questions and that the following information has been explained to me in language I understand. By signing this document I authorize the treatment described below and acknowledge the terms and conditions set forth in this consent.

Description of Treatment

Methadone is a long-acting opioid agonist used to treat opioid use disorder. The primary objectives of methadone treatment are to reduce opioid withdrawal symptoms, diminish cravings, block the euphoric effects of other opioids at appropriate doses, reduce illicit opioid use, and improve psychosocial functioning.

I understand that initial treatment typically begins with a supervised induction phase during which doses are adjusted to an effective maintenance dose. Dose adjustments may continue during treatment based on clinical assessment. Methadone will be dispensed only by authorized clinic staff and must be ingested under supervision unless specifically authorized as take-home doses according to clinic policy and applicable law.

Clinic/Program Name: Treating Clinician:

Risks, Benefits, and Alternatives

Benefits: Methadone has been demonstrated to reduce opioid withdrawal and cravings, decrease illicit opioid use, reduce the risk of infectious disease transmission, and improve social functioning and retention in treatment.

Risks: I understand and accept that methadone treatment carries risks including but not limited to sedation, respiratory depression (which can be fatal), arrhythmias, hypotension, constipation, allergic reactions, sexual dysfunction, and potential interactions with other central nervous system depressants including alcohol, benzodiazepines, and other opioids. I acknowledge that dose-related respiratory depression and overdose are possible, particularly in the context of concurrent sedative use.

Alternatives: I have been informed of reasonable alternatives, including medically supervised withdrawal (detoxification), buprenorphine treatment, naltrexone therapy, and non-pharmacologic treatments. I understand that refusal of methadone treatment will not affect my right to receive medically indicated emergency care.

Treatment Conditions and Monitoring

I consent to regular clinical assessments, observed dosing, and periodic urine or other biological specimen testing for drugs and medication adherence. Refusal to provide requested specimens or repeated positive tests for non-prescribed sedatives or illicit opioids may result in treatment modification, dose reduction, or discharge from the program in accordance with clinic policies.

I understand that clinic rules concerning conduct, attendance, counseling participation, and medication storage apply. I agree to attend scheduled appointments and comply with program requirements including psychosocial support or counseling when recommended.

I understand that take-home ("unsupervised") doses may be provided only when I meet clinical and program criteria and in accordance with applicable law. I agree to store all medications securely, out of reach of children, and not to share or sell medications.

Testing, Reporting and Release of Information

I authorize collection and testing of urine, blood, breath, or other specimens to monitor medication adherence and detect other substances. I understand that positive test results may be documented in my medical record and may be used in treatment planning.

I consent to the disclosure of relevant treatment information to other health care providers, pharmacies, and authorized entities for purposes of treatment, payment, or program operations as permitted by law. Specific additional releases of confidential information require separate written authorization.

Special Warnings and Advisories

Pregnancy: Methadone crosses the placenta. I understand that pregnancy may alter treatment considerations and that abrupt discontinuation during pregnancy can be harmful to both mother and fetus. I agree to notify my clinician immediately if I am pregnant, suspect I may be pregnant, or plan to become pregnant.

Concomitant sedatives: Combining methadone with benzodiazepines, alcohol, other opioids, or other central nervous system depressants increases the risk of life-threatening respiratory depression. I will inform my clinician of all prescriptions and over-the-counter medications, supplements, and recreational substance use.

Overdose prevention: I understand the signs of opioid overdose and the availability of opioid overdose reversal medication. I acknowledge that I have been offered training and information on overdose response and naloxone administration. I accept that carrying naloxone is recommended for myself and household members.

Patient Acknowledgments and Authorizations

I have read (or had read to me) and understand the information above. I have had the opportunity to ask questions and have received answers to my satisfaction. By signing below I consent to receive methadone treatment at this clinic and authorize the clinic to provide services as described.

I authorize release of relevant treatment information to my listed emergency contact and authorized recipient for coordination of care and in the event of a medication-related emergency: I authorize

I understand that this authorization will remain in effect until: unless I revoke it earlier in writing. I understand that revocation does not affect information already released in reliance on this consent.

Right to Withdraw and Program Policies

I understand that I have the right to withdraw consent at any time by providing written notice to the clinic, subject to clinical and legal obligations. Withdrawal of consent may result in discontinuation of methadone treatment and may require transition planning to alternative therapies or medically supervised withdrawal.

I acknowledge that noncompliance with clinic policies, diversion of medication, or behavior that jeopardizes safety may result in modification of treatment, including reduction or discontinuation of methadone.

What the Healthcare Methadone Treatment Consent Form Is

The Healthcare Methadone Treatment Consent Form documents a patient’s informed agreement to begin or continue methadone treatment for opioid use disorder. It records the risks, expected benefits, treatment plan, medication dosing schedule and patient responsibilities. The form establishes legal consent for clinical care, data sharing needed for treatment, and any use of controlled substances under applicable federal and state rules. Clinics typically combine clinical assessment sections with signature blocks for the patient and treating clinician to confirm understanding and voluntary consent.

Why a Formal Consent Form Matters in Methadone Care

A clear, signed consent form creates a documented record of informed choice, supports clinical decision-making, and helps meet regulatory obligations for opioid treatment programs.

Frequent Errors to Avoid When Preparing This Form

  • Using initials instead of a full signature for patient or clinician, leading to questions about consent authenticity and possible rejection by auditors.
  • Mismatching patient identity data (name or DOB) with medical records, which can delay treatment or trigger re-verification procedures.
  • Failing to document witness or notary details where state or program rules require them, resulting in noncompliant records.
  • Relying on an e-signature process that lacks an adequate audit trail or HIPAA protections, exposing the program to regulatory risk.

Consequences of Incorrect or Missing Consent

Clinical Risk: Increased overdose or adverse-event exposure
HIPAA Violation: Potential fines and corrective action
Regulatory Sanctions: Program penalties or license review
Civil Liability: Malpractice or negligence claims
Treatment Disruption: Administrative delays or care interruption
