Assessment & Consent

Description of Assessment and Procedures: The undersigned authorizes clinical evaluation and environmental assessment related to suspected mold exposure. Environmental assessment may include visual inspection, moisture mapping, and collection of air and/or surface samples for laboratory analysis. Clinical evaluation may include history, physical examination, diagnostic testing, and documentation of symptoms attributable to possible environmental exposure.

Risks and Benefits: Environmental sampling is generally low risk; however, minor disruption to facility surfaces or temporary disturbance of settled dust may occur. Clinical evaluation may involve diagnostic maneuvers that carry routine risks appropriate to the test. Benefits include identification of environmental conditions that may contribute to symptoms and documentation for medical care or remediation planning.

Right to Withdraw: Authorization for sampling and release may be withdrawn at any time by providing written notice to the healthcare provider, except to the extent that action has already been taken in reliance on this authorization. Withdrawal will not affect information already disclosed under this authorization.

No Guarantee of Remediation: Sampling and clinical assessment do not guarantee the identification or elimination of all mold or allergens. Results of environmental testing are interpretive and should be considered with clinical findings and building inspection results.

Authorization to Share Medical Information: I authorize release of relevant medical information and assessment results to the following third parties (e.g., building owner, environmental consultant, employer) for the purpose of coordinating remediation and medical follow-up.

HIPAA & Authorization

I acknowledge that protected health information (PHI) related to this evaluation may be disclosed to the authorized recipients listed above for the purpose of environmental assessment, remediation coordination, and medical follow-up. This authorization is voluntary and is not a condition of treatment, enrollment, or eligibility for benefits.

Authorization Expiration: This authorization will expire on unless earlier revoked in writing.

Patient Certification: I certify that the information provided on this form is true and correct to the best of my knowledge. I understand the scope and purpose of the assessment, the risks described above, and my right to withdraw authorization as set forth above.

HIPAA Acknowledgement: I acknowledge receipt of the facility's privacy practices and understand how my information may be used and disclosed as described above.
