Consent for Neurosonology Examination

I authorize the performance of the neurosonology examination(s) indicated above. The procedure will involve application of ultrasound transducers to the head, neck, and/or temporal windows, recording of Doppler signals, and capture of images. If contrast-enhanced techniques are indicated, contrast agents may be administered intravenously to improve diagnostic accuracy.

I understand the following reasonably anticipated risks and limitations: transient discomfort at the probe site, allergic or adverse reaction to contrast agents, inability to obtain diagnostic images due to anatomical or technical factors, and rare risk of infection or bruising at an IV site. I have been given the opportunity to ask questions and understand that no guarantee has been made regarding the results.

I understand I may withdraw consent at any time prior to or during the procedure without penalty, but withdrawal may prevent completion of the examination and could limit diagnostic evaluation by my physician.

Authorization Expiration Date (if different from date of signature):

HIPAA / Privacy Acknowledgment

I acknowledge that I have been provided with the facility's privacy practices and understand that my medical information, including images and reports generated from this examination, will be used and disclosed for treatment, payment, and healthcare operations as permitted by law. I permit the release of findings to my referring physician and other treating providers involved in my care.

I consent to the use of de-identified images for education and quality assurance unless I explicitly withhold such use below.

What the Healthcare Neurosonology Form Is

The Healthcare Neurosonology Form is a standardized clinical report used to document neurovascular ultrasound examinations, including carotid duplex, transcranial Doppler, and cerebrovascular flow studies. It captures patient identifiers, clinical indication, technical parameters, measurements, image references, findings, and an interpreted impression. The form supports consistent documentation for diagnosis, treatment planning, billing, and quality review. When completed correctly it links images, diagnostic codes, ordering provider details, and signatures or attestations required for the medical record.

Why a Standardized Neurosonology Form Matters

A consistent form reduces diagnostic variability, supports compliance with medical record requirements, and speeds clinician review by organizing key findings and impressions in a predictable layout.

Why a Standardized Neurosonology Form Matters

Who Typically Completes or Reviews This Form

Typical users span clinical, administrative, and technical roles involved in neurovascular imaging.

  • Sonographers and vascular technologists who perform the exam and enter technical measurements and image references.
  • Interpreting physicians (radiology, vascular neurology) who review images, write the impression, and sign the report.
  • Health information management staff and coders who verify documentation for billing and retention.

Clear role separation improves data accuracy and reduces downstream edits or billing denials.

Core Sections Found on a Professional Neurosonology Form

A complete form separates administrative data, technical parameters, measured values, narrative findings, a concise impression, and ordering or follow-up recommendations.

Patient Details

Full legal name, date of birth, medical record number, and unique identifiers to match images and orders precisely for the medical record.

Clinical Indication

Reason for exam and relevant history such as stroke symptoms, TIA, or vascular risk factors to guide interpretation and coding.

Technical Parameters

Exam type, transducer frequency, Doppler angles, machine presets, and image quality notes that affect measurement reliability and reproducibility.

Measured Findings

Peak systolic and end-diastolic velocities, ratio calculations, waveform descriptions, and plaque morphology with image references for each vessel segment.

Impression

Concise diagnostic summary with clinically actionable statements, degree of stenosis where applicable, and recommended follow-up or further imaging.

Signatures & Codes

Interpreting clinician signature, date/time, CPT/ICD codes for billing, and ordering provider information for legal and reimbursement records.

Security and Compliance Checklist

Protected Health Information: Include only necessary PHI
Encryption: TLS 1.2/1.3 in transit
Audit Trail: Maintain timestamped logs
Business Associate Agreement: BAA required for vendors
Access Controls: Role-based permissions
Data Integrity: Tamper-evident storage

Step-by-Step: Completing the Neurosonology Form

Follow these sequential steps at the point of care to ensure a complete, compliant report.

  • 01
Verify Patient: Confirm name, DOB, and MRN before imaging.
  • 02
Record Demographics: Enter identifiers and order details precisely.
  • 03
Capture Images: Acquire standardized views and document measurements.
  • 04
Finalize Report: Complete findings, impression, and signer authentication.

Recommended Digital Workflow Settings

Configure the form workflow in your EHR/PACS to reduce manual entry and enforce required fields.

Field Configuration
Patient Matching EMR ID lookup
Template Neurosonology standard
Authentication Two-factor sign
Storage PACS and EHR

How Electronic Completion and Routing Works

A typical eWorkflow links the formatted form with images, routes for review, and records the audit trail automatically.

  • Upload Form: Attach exam template to order.
  • Place Fields: Add required sign, date, and text fields.
  • Send to Interpreter: Route to physician for review.
  • Sign & Archive: Signer authenticates and file archives.

Platform and Integration Considerations

Ensure your signing and storage platform supports PHI protection, audit logs, and integrations with clinical systems.

  • Integrations: Support for EHR, PACS, and cloud storage
  • Authentication: Two-factor and enterprise SSO options
  • Formats: PDF, DICOM links, and structured data exports

Typical Timeframes and Turnaround Expectations

Timeframes vary by facility and clinical urgency; document expectations to meet clinician and billing needs.

Routine Report Turnaround:

Key Processing Milestones from Order to Archive

Track milestones to ensure the study moves efficiently from scheduling to final archiving and coding.

01

Order Placed

Scheduling and indication captured.

02

Study Performed

Images and preliminary measurements recorded.

03

Preliminary Read

Immediate findings documented for care team.

04

Final Report

Interpreting physician signs and archives report.

Common Preparation and Documentation Errors

  • Incorrect patient identifiers leading to mismatched records and potential care delays.
  • Incomplete hemodynamic measurements or missing waveform documentation that prevent accurate stenosis grading.
  • Vague clinical history entries that impede correct interpretation and may affect coding.
  • Unsigned or undated reports that require addenda and complicate billing or legal review.

Risks and Consequences of Incorrect Documentation

Misidentification: Clinical risk
Missing Consent: Legal exposure
HIPAA Breach: Regulatory fines
Incomplete Findings: Inaccurate diagnosis
Billing Errors: Claim denials
Med-Legal Risk: Liability for care decisions
