Description of Services: The Provider will deliver clinical services limited to the scope described below. Either party may terminate this Agreement with seven (7) days' written notice. Termination does not relieve the Patient of obligations to pay for services already rendered.

Liability, Indemnity and Governing Law

Provider will perform services in accordance with applicable professional standards. To the extent permitted by law, each party releases the other from liability for incidental damages and agrees to indemnify for third-party claims arising from breach of this Agreement or negligent acts. This Agreement is governed by the laws of the state where the Provider practices.
