Provider will render healthcare services as clinically indicated and agreed between Provider and Patient. The specific services to be provided are described as:

Patient hereby authorizes Provider to perform the services described above. Patient acknowledges that no guarantee of outcome has been made and that services may involve risks, including but not limited to infection, adverse reaction, or other complications specific to the procedure or treatment. Patient further acknowledges that alternatives, associated risks, and expected benefits have been explained and that Patient has had the opportunity to ask questions.

PAYMENT, BILLING, AND FINANCIAL RESPONSIBILITY

Patient agrees to be financially responsible for all charges for services not paid by insurance or other third-party payers. Provider will bill Patient's insurance as a courtesy where permitted; however, Patient remains responsible for co-payments, deductibles, co-insurance, non-covered items, and any portion denied by the insurer. Patient authorizes Provider to submit claims and to release information necessary for billing and payment.

Payment arrangements, missed appointment fees, and collection procedures are governed by Provider policies. Charges unpaid after reasonable collection efforts may be referred to collections and may incur additional fees and interest as permitted by law.

AUTHORIZATION TO RELEASE AND RECEIVE MEDICAL INFORMATION

Patient authorizes Provider to disclose and obtain medical and billing information to and from other health care providers, insurers, and designated persons as reasonably necessary to provide care and to process claims. This authorization includes release of records related to diagnosis, treatment, payment history, and related documentation.

PATIENT RIGHTS, PRIVACY, AND HIPAA ACKNOWLEDGMENT

Patient acknowledges receipt of Provider's Notice of Privacy Practices and understands that the notice describes how medical information about Patient may be used and disclosed and how Patient can obtain access to this information. Patient may request restrictions on certain uses and disclosures, and Provider will comply where required by law.

Patient specifically authorizes the use of protected health information for treatment, payment, and health care operations, subject to the limits set forth in Provider's privacy policies and applicable law.

Acknowledge HIPAA Privacy Notice:

REVOCATION, WITHDRAWAL, AND TERMINATION

Patient may revoke this Contract or any authorizations herein in writing, except to the extent that Provider has already relied upon the authorization or to the extent that action has been taken in reliance on this Contract. Either party may terminate this Contract for convenience with reasonable notice; termination does not affect obligations accrued prior to termination.

LIMITATION OF LIABILITY AND DISPUTE RESOLUTION

To the extent permitted by law, Provider's liability for any claim arising from treatment shall be limited to direct damages and shall exclude consequential, incidental, or punitive damages. Parties agree to attempt good faith resolution of disputes. If a dispute cannot be resolved informally, the parties agree to pursue mediation or arbitration as set forth in Provider policy or as otherwise mutually agreed in writing.

REPRESENTATIONS AND WARRANTIES

Patient represents that all information provided is true and complete to the best of Patient's knowledge. Patient agrees to promptly notify Provider of any changes to insurance coverage, contact information, or health status that may affect care or billing.

ADDITIONAL AUTHORIZATIONS

Patient consents to leaving appointment reminders and limited clinical information on voicemail or by text message at the contact numbers provided unless Patient indicates otherwise in writing. Patient consents to Provider obtaining prior records from other providers for continuity of care.

CONSENT FOR MINOR OR INCAPACITATED PATIENTS

If the patient is a minor or lacks legal capacity, the undersigned certifies that they are the parent or legal guardian with authority to consent to treatment and execution of this Contract on behalf of the patient. The signer agrees to assume financial responsibility as set forth above.
