Procedure Description, Risks and Alternatives

I authorize the licensed practitioner to perform a lash lift procedure. A lash lift is a cosmetic treatment that modifies the curvature and position of natural eyelashes using lifting and setting solutions. The procedure may include cleansing, application of adhesive and permutation/setting solutions, and application of tint if requested.

Acknowledged risks include, but are not limited to: chemical irritation, redness, swelling, allergic reaction, transient or permanent eyelash damage or loss, uneven lift, lash brittleness, ocular surface injury if products contact the eye, infection, and pigmentary changes. Results vary and are not guaranteed. Alternatives include, but are not limited to: no treatment, eyelash extensions, or other cosmetic eyelash treatments.

I understand that patch testing does not eliminate the risk of allergic reaction and that reactions can occur despite prior negative tests. I consent to removal of contact lenses prior to treatment and will inform the practitioner of any new eye symptoms prior to the procedure.

If patch test performed, date:

Pre- and Post-Care, Photographs

I have been informed of pre- and post-care instructions, including avoiding water, steam, saunas, and eye rubbing for the recommended period following treatment, and using recommended aftercare products. Failure to follow aftercare instructions may adversely affect the results.

What the Healthcare Patient Lash Lift Consent Form Is

The Healthcare Patient Lash Lift Consent Form documents a patient's informed consent for a cosmetic lash lift procedure performed in a clinical or licensed salon setting. It captures patient identity, relevant medical and ocular history, allergies, and specific risks associated with lifting solutions and adhesives. The form confirms that the patient received verbal and written aftercare instructions and has had an opportunity to ask questions. In healthcare contexts this record also supports HIPAA-compliant handling of protected health information and serves as an auditable legal record of authorization.

Why a Standardized Consent Form Matters

A consistent Healthcare Patient Lash Lift Consent Form protects both patient and provider by clearly documenting informed consent, outlining risks and aftercare, and creating a dated, signed record that supports risk management and regulatory compliance.

Why a Standardized Consent Form Matters

Who Typically Completes This Consent Form

Providers, estheticians, and clinical staff use this form to document patient consent, collect medical history, and record risks and aftercare instructions.

  • Licensed estheticians performing cosmetic lash lift procedures in salons or med-spa settings.
  • Ophthalmologists or dermatology clinics offering cosmetic lash services with medical oversight.
  • Clinic administrative staff who retain signed consent forms for recordkeeping and compliance.

The form should be completed prior to any lash lift procedure and retained in the patient record for compliance and continuity of care.

Representative Roles and Responsibilities

Provider — Licensed Esthetician

Providers must confirm patient identity, review ocular and systemic medical history, disclose chemical and adhesive risks, obtain clear written consent, and retain the signed form in the clinical record while following HIPAA safeguards for protected health information.

Patient — Individual

Patients should disclose allergies, contact lens use, prior reactions to cosmetics, and any eye conditions; review risks and aftercare instructions carefully; ask clarifying questions; and provide a dated signature to confirm informed consent before treatment.

Essential Sections to Include on the Form

A professional Healthcare Patient Lash Lift Consent Form groups information into clear sections that establish identity, medical suitability, treatment details, risks, aftercare, and signature confirmation.

Patient Identification

Full legal name, date of birth, contact information, and an identifier such as a patient or file number to ensure records are linked correctly and to prevent misidentification during follow-up care.

Medical and Ocular History

Questions about eye disease, recent eye surgery, skin sensitivities, pregnancy, breastfeeding, and medications that may affect healing or increase risk of adverse reaction to lifting solutions or adhesives.

Allergies and Reactions

Explicit prompts to list known drug, adhesive, or cosmetic allergies and prior adverse reactions so the provider can select safe products or perform a patch test where appropriate.

Procedure Details

Description of the lash lift technique, chemicals and adhesives to be used (brand/generic where possible), expected outcomes, and foreseeable complications such as irritation, lash breakage, or infection.

Aftercare and Limitations

Clear aftercare instructions, activity restrictions, contact guidance for complications, and realistic expectations about results and durability to reduce disputes and support safe recovery.

Signature and Consent

An explicit consent statement with dated signature lines for the patient (or authorized guardian) and provider, and a space to note any questions asked or materials provided.

Step-by-Step: Filling Out and Recording the Form

Follow these steps to complete and record a Lash Lift Consent Form accurately and securely for each patient encounter.

  • 01
Verify Identity: Compare government ID with form name and DOB before proceeding.
  • 02
Collect Medical History: Ask targeted questions about ocular health, medications, and allergies.
  • 03
Explain Risks: Describe possible adverse effects, aftercare, and alternatives in plain language.
  • 04
Sign and Store: Obtain signatures, date the form, and save to the patient record securely.

Configuring an Online Consent Workflow

When digitizing the form, configure fields and routing to ensure accurate data capture and secure storage.

Field Configuration
Patient ID Field Required; validate format and prevent duplicates
Medical History Section Use conditional fields to show relevant follow-ups
Signature Field Enable typed or drawn signature and timestamp
Storage Destination Save to EHR or secure cloud with access controls

Typical Submission and Record Flow

A consistent electronic flow reduces friction and ensures each signed form is captured and retrievable.

  • Upload Form: Provider uploads template to the signing platform.
  • Assign Signer: Add patient email or use in-clinic tablet for signing.
  • Authenticate: Use email link, SMS code, or stronger verification where required.
  • Archive: Store signed PDF and audit trail in patient record.

Technical and Integration Considerations

Choose a platform that supports secure PDF signing, audit trails, and integrations with your record system.

  • Formats: PDF, DOCX supported
  • Integrations: Connects with EHR and cloud storage
  • Authentication: Email, SMS, or advanced options

Ensure the selected vendor supports HIPAA BAAs if you handle protected health information and that signed documents are retained with tamper-evident audit logs for legal defensibility.

Timing: When to Collect and Record Consent

Collect consent before any lash lift begins and record the execution date; some follow-up and retention timelines are time-sensitive.

Before Procedure:

Obtain signed consent on the day of service or earlier during pre-screening.

At Visit:

Confirm no new contraindications before starting treatment.

Record Retention:

Keep the signed form with the patient record per retention rules.

HIPAA Access:

Provide copies upon authorized patient request within statutory timeframes.

Revocation Window:

Patients may withdraw consent before treatment; document any withdrawal immediately.

Key Milestones from Intake to Record Closure

Track these sequential milestones to confirm completion and ongoing record maintenance for each patient consent.

01

Intake and Screening

Complete medical questionnaire and identify contraindications prior to scheduling.

02

Consent Execution

Patient signs and dates the consent immediately before the procedure.

03

Treatment and Documentation

Provider documents products used and any intra-procedure notes.

04

Archive and Retention

Common Errors to Avoid

  • Failing to verify identity or link the form to the correct patient file can create medical record mismatches and billing complications.
  • Omitting allergy or contact lens information increases risk of adverse reactions and potential professional liability for the provider.
  • Using vague or technical language in risk descriptions can leave patients unclear about potential outcomes and reduce the strength of informed consent.
  • Storing signed forms without access controls or audit logs risks HIPAA violations and makes reconstructing the consent timeline difficult.

Consequences of Poor or Missing Consent

Invalid Consent: Procedure may be legally void
HIPAA Breach: Unauthorized PHI disclosure penalties
Medical Liability: Increased malpractice or negligence risk
Regulatory Action: State board investigation possible
Civil Litigation: Patient claims and damages
Service Refusal: Provider may decline treatment
