2. Effect on Benefits and Revocation: The Patient acknowledges that assignment of pension benefits may affect the amount of benefits available for the Patient's personal use. The Patient may revoke this Attachment in a signed, written statement delivered to the Provider and the Pension Administrator; however, revocation will not affect payments made in reliance on this Attachment prior to receipt of the revocation by the Pension Administrator.

3. Governing Law; Remedies: This Attachment shall be governed by the laws governing the pension plan and, to the extent not preempted by federal law, the laws of the jurisdiction in which the Provider's principal office is located. Any disputes arising under this Attachment shall be resolved in accordance with applicable plan procedures and law.

4. No Guarantee of Payment: The Patient understands that this Attachment does not guarantee payment by the Pension Administrator. The Provider retains the right to pursue other collection remedies consistent with applicable law where pension benefit payment is not available or permitted.

Privacy and HIPAA Acknowledgment

The undersigned acknowledges receipt of the Provider's Privacy Practices and specifically authorizes the disclosure of protected health information to the Pension Administrator and plan fiduciaries to the extent necessary to process benefit determinations or payments under this Attachment. This authorization is limited to information reasonably necessary to accomplish the stated purpose and is subject to applicable law.

Unless an earlier date is specified above, this Authorization shall expire one year from the Effective Date or when the Patient revokes the Authorization in writing, whichever occurs first.

Additional Instructions to Pension Administrator

If direct payment to the Provider is permitted under the plan, Pension Administrator is authorized to remit payment to the Provider at the billing address identified above. All payments made in reliance on this Attachment shall be applied to the Provider's account for the services identified herein.
