What a Healthcare Pharmacy Mentor Agreement Covers

A Healthcare Pharmacy Mentor Agreement is a written contract that sets expectations between a licensed pharmacist acting as mentor and a mentee such as a pharmacy resident, intern, or new staff pharmacist. The document defines scope of mentoring activities, supervision levels, performance goals, timeframes, confidentiality and information-sharing rules, and any compensation or expense reimbursements. For clinical settings it commonly includes HIPAA-related data handling language and a statement of applicable professional licensure. This agreement helps reduce ambiguity about responsibilities and provides a record for credentialing, HR, and compliance reviews.

Why a Formal Mentor Agreement Matters

A clear written agreement aligns expectations, documents supervisory responsibilities, and helps protect patient privacy where PHI is involved.

Why a Formal Mentor Agreement Matters

Who Typically Completes This Agreement

The agreement is used by individuals and organizations involved in pharmacy training and supervision.

  • Licensed pharmacist mentors: Supervising pharmacists, residency preceptors, and clinical specialists responsible for professional oversight.
  • Mentees and trainees: Pharmacy residents, interns, newly hired pharmacists, or technicians in training programs.
  • Employers and HR: Pharmacy managers, credentialing staff, and educational program coordinators who maintain records and ensure compliance.

Parties should keep a signed copy in the personnel file and make it available for audits, licensing, or credentialing reviews.

Core Sections to Include in a Professional Agreement

A strong mentor agreement is organized to make responsibilities, timelines, and protections explicit for both parties and for institutional review.

Parties and Credentials

Identify mentor and mentee by full legal name, professional license type and number, employer, and contact information to ensure credential verification and accurate records.

Effective Term

State the agreement start date and end date, plus renewal or extension terms so obligations and review triggers are unambiguous.

Scope of Mentoring

List specific activities, clinical settings, expected hours, supervision level, learning objectives, and any competency milestones to be achieved.

Duties and Reporting

Define mentor duties (feedback cadence, evaluations) and mentee responsibilities (attendance, documentation), plus escalation or incident reporting procedures.

Confidentiality and HIPAA

Include HIPAA-compliant confidentiality language and any institutional privacy addenda governing patient information and permissible disclosures.

Compensation and Expenses

Clarify stipend, honoraria, expense reimbursement, or release time; specify invoicing and payment terms if applicable.

Common Preparation Mistakes to Avoid

  • Using informal language that leaves mentoring duties vague or unenforceable, such as 'assist as needed' without measurable goals.
  • Failing to verify mentor licensure and state disciplinary status, which can invalidate supervisory authority in some institutions.
  • Omitting HIPAA acknowledgements or required institutional privacy addenda when mentees will access patient records.
  • Not retaining a signed copy in both the personnel file and the clinical credentialing system, complicating audits or accreditation reviews.

Consequences of a Deficient Agreement

Invalid Authorization: May void supervisory authority
HIPAA Violation: Risk of breach penalties
Professional Discipline: Licensure complaints or sanctions
Operational Delay: Credentialing or onboarding hold
Financial Exposure: Reimbursement or stipend disputes
Recordkeeping Gaps: Audit findings and remediation
