I authorize the physiotherapy provider and authorized staff to perform evaluation, manual therapy, therapeutic exercise, electrotherapy, dry needling (if applicable), and other modalities as clinically indicated. The anticipated plan of care is:

Risks and Benefits: I understand that physiotherapy has known benefits including pain reduction, improved mobility, and functional gains, and involves certain risks including but not limited to increased pain, muscle soreness, bruising, strain, aggravation of pre-existing symptoms, and rare complications such as injury. No guarantee of specific outcome is made. I have had the opportunity to ask questions and understand the nature, risks, benefits, and alternatives.

I voluntarily consent to the proposed physiotherapy evaluation and treatment modalities and authorize competent licensed clinicians to provide care in accordance with the plan above.

Financial Responsibility

I acknowledge responsibility for all charges for services rendered by the provider. Insurance billing will be submitted as a courtesy; however, I am financially responsible for any co-payments, deductibles, coinsurance, non-covered services, and amounts denied by insurance.

Assignment of Benefits: I authorize payment of insurance benefits to the provider for services rendered and permit release of medical information necessary to process claims. This assignment does not relieve me of financial responsibility for unpaid balances.

Cancellation and No-Show Policy

I agree to provide at least 24 hours' notice for cancellation of scheduled appointments. Failure to provide timely notice or failure to appear for an appointment may result in a cancellation fee, for which I am financially responsible. Repeated missed appointments may be grounds for termination of care.

Authorization to Release or Exchange Information

I authorize the provider to release or obtain medical information as necessary to coordinate care with other healthcare providers, insurers, or my employer when required for treatment or payment. This authorization is valid until the expiration date indicated or until revoked in writing.

Termination of Services & Liability

Either party may terminate services upon written notice. The provider may terminate care for noncompliance, failure to pay, or repeated missed appointments. The provider will take reasonable measures to ensure patient safety but is not liable for unforeseen complications arising from treatment when treatment was provided in compliance with accepted professional standards.

I understand that in the event of an emergency, the provider will arrange for or provide emergency care as reasonably necessary and will notify my emergency contact when appropriate. I accept responsibility for emergency treatment costs that may be incurred.
