The Provider may suspend or terminate the protocol if continuation would, in the Provider's professional judgment, jeopardize the Patient's health.

Authorization Period and Modification

This authorization to implement the described protocol is effective as of the date of signature below and shall expire on: unless earlier revoked in writing or superseded by a revised protocol signed by the Patient.

Any modification to the protocol that materially affects risk or scope must be documented in writing and agreed to by the Patient.

Liability, Indemnification, and Governing Law

The Provider will exercise reasonable care in applying the protocol. The Patient acknowledges that no medical procedure is without risk. To the extent permitted by law, the Patient agrees to release and hold harmless the Provider from liability for ordinary negligence in connection with the performance of the protocol, provided the Provider acts within accepted standards of care. The Patient agrees to indemnify the Provider for claims arising from the Patient's failure to follow protocol instructions.

This Agreement will be governed by the laws of the jurisdiction where the Provider is located. Disputes will be addressed first through good faith negotiation and, if unresolved, through mediation before resorting to formal litigation, except where injunctive relief is sought.
