Purpose

This policy establishes requirements, responsibilities, and procedures for planning, approving, implementing and evaluating care delivery redesign initiatives that materially affect clinical processes, patient safety, or patient access. Redesign activities include workflow redesign, technology-enabled care model changes, staffing model adjustments, and physical space reconfiguration.

Scope

Applies To

Definitions

Redesign Initiative: a defined project to change clinical or operational processes that may affect patient care, safety, access, or privacy. Pilot: a controlled, time-limited implementation to evaluate effects prior to full deployment. Patient Representative: an individual designated to provide patient perspective during design and testing.

Policy Statement

All redesign initiatives that may materially affect clinical care, patient safety, patient access, or the use of protected health information must comply with this policy. No redesign that alters clinical responsibilities, care pathways, or patient-facing processes may proceed to pilot or implementation without documented approval by the Executive Sponsor and completion of the required risk assessment, privacy review, patient impact assessment, training plan, and monitoring plan.

Roles and Responsibilities

Redesign Process Requirements

Each initiative must include the following documented elements prior to pilot approval: assessment of current state; proposed changes and measurable objectives; patient safety risk assessment; privacy/data protection impact analysis; staffing and training plan; communications plan for patients and staff; pilot criteria and success metrics; and a rollback/mitigation plan.

Pilot and Evaluation

Pilots must be limited in scope and duration, with predefined stop/go criteria and validated measurement methods. Adverse events or unanticipated patient harm during pilot must trigger immediate suspension and review.

Training and Communication

All staff affected by a redesign must complete required training prior to deployment. Patient-facing communications must be prepared and approved by the Communications Lead and the Clinical Lead prior to release.

Patient Impact Assessment

Will redesign require patient consent or notification?:

Documentation, Record Retention and Compliance

All documentation related to redesign initiatives, including approvals, risk assessments, training records, pilot data and communications must be retained in accordance with organizational record retention policy and applicable law. Noncompliance with this policy may result in corrective action up to and including suspension of the initiative.

Review and Revision

This policy will be reviewed at least annually or sooner following significant regulatory change, adverse patient outcomes linked to redesign, or upon request by the Executive Sponsor. Revisions must be approved and documented in the change history.

Certification

By signing below the authorized signatory certifies that the information provided in this document is complete and accurate to the best of their knowledge, that required risk and privacy reviews have been completed, and that the initiative will be conducted in accordance with applicable laws, regulations, and organizational policies. The signatory further acknowledges responsibility for ensuring monitoring of outcomes and that corrective action will be taken if the redesign adversely affects patient safety or care quality.

What the Healthcare Redesign Policy Is and Covers

A Healthcare Redesign Policy is an organizational document that defines the governance, scope, and procedures for systematic changes to clinical workflows, patient access, data handling, and administrative processes. It establishes roles, decision authorities, performance metrics, and change-control steps to ensure safety, continuity of care, and regulatory compliance. The policy typically addresses stakeholder engagement, pilot testing, training, privacy safeguards, recordkeeping, and mechanisms for monitoring outcomes and addressing unintended effects on patients or staff.

Why a Clear Redesign Policy Matters

A formal policy reduces implementation risk, clarifies accountability, and preserves patient safety while enabling measurable improvements in throughput, quality, and cost control. It documents compliance steps and audit trails needed for regulatory review and continuous improvement.

Why a Clear Redesign Policy Matters

Primary Teams That Use the Healthcare Redesign Policy

The policy is used by cross-functional teams that design, approve, and operate redesigned care pathways and administrative processes.

  • Clinical leadership and physicians who approve care-pathway changes and validate clinical safety.
  • Quality, safety, and compliance officers who confirm regulatory alignment and document audit trails.
  • Operations and IT teams who implement workflows, integrate systems, and maintain version control.

Clear role assignments in the policy help reduce delays, limit scope creep, and support defensible compliance records during audits.

Core Components to Include in a Professional Policy

A strong Healthcare Redesign Policy combines governance, process mapping, privacy controls, performance measurement, training, and revision procedures into a single reference that guides every redesign effort.

Scope & Objectives

Define clinical areas, patient populations, and operational goals with measurable KPIs and success criteria tied to safety and quality outcomes.

Governance

Assign decision rights, steering committee membership, approval thresholds, and escalation paths for exceptions and urgent changes.

Workflow Mapping

Document baseline and redesigned workflows with roles, handoffs, dependencies, and required IT integrations to ensure operational clarity.

Privacy & Security

Describe data classification, access controls, encryption, BAAs where applicable, and procedures to protect PHI under HIPAA rules.

Training & Change

Specify training cadence, competency checks, communication plans, and mechanisms for collecting frontline feedback during pilots.

Measurement & Review

Define metrics, reporting cadence, post-implementation reviews, and scheduled policy revision intervals with version history.

Step-by-Step: Implementing a Redesigned Process

Follow a staged approach from assessment through review to limit disruption and capture learning from each phase.

  • 01
Assemble Stakeholders: Form a cross-functional team including clinicians and compliance.
  • 02
Map Current State: Document existing steps, delays, and failure points.
  • 03
Design & Pilot: Create a small-scale pilot with monitoring and rollback plans.
  • 04
Approve and Scale: Obtain sign-offs, train staff, and expand with metrics tracking.

Digital Workflow Settings to Configure

When automating policy-driven tasks, configure routing, authentication, retention, and notifications to match governance requirements.

Workflow field configuration header row Workflow Element | Configuration
Owner Department name | Responsible approver role
Signature Order Sequential or parallel | Role-based routing
Authentication Email, SMS code, or stronger MFA
Retention Settings Archive period | Access and export rules

Technical Requirements for eSubmission and Integration

Effective online implementation requires integrations with EHRs, identity providers, and secure storage to preserve privacy and traceability.

  • Integrations: EHR, CRM, cloud storage, and ERP connectors
  • File formats: PDF, DOCX, and exported audit logs
  • Authentication: Email, SMS codes, SSO and MFA options

How Digital Policy Approval Typically Flows

A consistent electronic routing sequence preserves order, records signatures, and triggers post-approval actions automatically.

  • Prepare Document: Upload template and add required fields.
  • Assign Roles: Specify signers and approval order.
  • Distribute: Send secure links or email invitations.
  • Archive: Store signed copy and audit trail.

Security and Compliance Controls to Require

Encryption: TLS 1.2/1.3 in transit, AES-256 at rest
Audit Trail: Timestamps, IP, and action log retained
HIPAA BAA: Business Associate Agreement required for PHI
21 CFR Part 11: Controls for regulated electronic records
ESIGN / UETA: Legal framework for e-signature validity
Access Controls: Role-based permissions and SSO support

Key Risks and Consequences of Poor Execution

Privacy Breach: HIPAA fines and corrective action plans
Operational Delay: Service interruptions and patient access impacts
Regulatory Noncompliance: Audit findings and potential sanctions
Contract Disputes: Liability for unmet obligations
Data Integrity Issues: Lost evidence for clinical decisions
Reputational Harm: Erosion of patient and payer trust

Typical Timelines and Key Deadlines During Redesign
