  1. Repairs will be billed first to the insurance policy identified above when coverage reasonably applies. If coverage is denied or not applicable, I accept responsibility for payment of parts and labor not otherwise covered.
  2. Facility or provider will attempt to obtain prior authorization where required by the insurer; obtaining prior authorization is not a guarantee of payment.
  3. Estimates provided are approximate and may change based on further diagnostics or discovery of concealed damage.

I acknowledge that repairs involve technical work and that residual risk exists including, but not limited to, diminished performance, incompatibility with aftermarket parts, or inability to restore original specifications.

I release the facility and its employees and contractors from liability for incidental or consequential damages arising from failure to restore equipment to original condition, except in cases of gross negligence or willful misconduct. I further acknowledge that the facility does not warrant third-party parts beyond the manufacturer's warranty offered with such parts.

I understand I may withdraw this authorization at any time prior to commencement of repair work by notifying the requesting staff. Withdrawal after work has commenced does not relieve me of responsibility for costs already incurred.
