By signing below, Patient Name: agrees to the following terms concerning payment for services rendered by this provider.

1. Financial Obligation: The patient (or responsible party) is financially responsible for all charges for services provided, including services not covered by insurance. Co-payments, co-insurance, and deductibles are due at the time of service.

2. Assignment of Benefits: The patient authorizes assignment of insurance benefits to the provider and authorizes the release of medical information to process claims. Patient hereby assigns to the provider any insurance benefits otherwise payable to the patient for services rendered.

3. Billing and Collections: If payment is not received when due, patient agrees to pay reasonable collection costs, attorney fees, and court costs incurred to obtain payment. Returned checks will incur a returned-check fee.

4. Verification of Coverage: The provider will attempt to verify insurance coverage; however, verification does not guarantee payment. Final determination of patient responsibility may occur after claims are processed.

5. Payment Arrangements: Payment plans may be available upon request. Requests must be documented in writing and approved by billing prior to any extended payment period.

Consent to Treatment and Release

I consent to and authorize the provider and its staff to provide medical diagnosis, treatment, and necessary health care services as deemed advisable by the treating practitioners. I understand that all treatments involve risks and potential complications and that no guarantees have been made regarding results.
