I acknowledge that participation in the retreat activities may involve inherent risks, including but not limited to physical exertion, therapeutic exercises, outdoor activity, group work, and travel between sites. I certify that I am physically and mentally able to participate. This authorization is voluntary and may be revoked in writing, except to the extent that action has already been taken in reliance on it.
