Care, Medication, and Emergency Treatment

In the event of an emergency, Facility is authorized to obtain emergency medical treatment for the resident when the resident or designated decision-maker cannot be reached. The resident or representative will be responsible for costs associated with emergency care and transportation.

Resident Responsibilities & House Rules

Resident agrees to comply with facility policies, to pay fees when due, to maintain personal hygiene and to respect other residents' rights. Facility may impose reasonable rules of conduct and may relocate resident within the facility for clinical or operational reasons with prior notice when practicable.

Termination, Refunds and Transfer

This Agreement may be terminated by Facility for nonpayment, material breach, or when Facility cannot meet the resident's clinical needs. Resident may terminate with thirty (30) days written notice unless otherwise stated. Refunds of prepaid fees and the security deposit will be calculated per Facility policy. Notice period (days):
