7. Medical Records Release and Authorization

Patient authorizes Provider to obtain and release medical records as necessary for treatment and payment. Any release beyond treatment, payment, or health care operations will be provided only upon separate written authorization.

8. Termination and Cancellation

Either party may terminate this Agreement for convenience upon written notice. Provider may terminate or limit services if Patient fails to comply with treatment recommendations, misses appointments in violation of Provider policy, or fails to pay charges when due. Billing obligations for services rendered prior to termination survive termination.

9. Liability, Indemnification and Dispute Resolution

Provider will exercise reasonable care in the provision of services. Except as prohibited by applicable law, Patient agrees to indemnify and hold harmless Provider, its employees and agents from any claims arising from Patient's failure to follow medical advice, provide accurate information, or from willful misconduct. To the extent permitted by law, Provider's total liability for professional negligence shall be limited to the fees paid for the services that are the subject of the claim.

All notices required under this Agreement shall be in writing and delivered to the addresses below.

11. Miscellaneous

This Agreement constitutes the entire agreement between the parties regarding the subject matter and supersedes prior understandings. Any amendment must be in writing and signed by both parties. If any provision is held unenforceable, the remaining provisions remain in full force and effect.
