Conditions of Approval

By signing below, the Design Firm and Facility hereby acknowledge and agree that approval is limited to the items and documentation specifically described in this form and its attached submission package. Approval certifies that, to the best of the signatory's knowledge, the submitted design documents substantially comply with the facility's design criteria and the listed code, safety and infection control items. Approval does not relieve the Design Firm of responsibility to produce final construction documents that fully comply with all applicable codes, regulations and standards, nor does it transfer permitting responsibility away from the Design Firm or Contractor where local law requires.

The Design Firm warrants that systems and materials specified in the approved documents shall be designed in accordance with applicable professional standards and shall coordinate interfaces with medical equipment, medical gases, electrical and data systems. The Facility shall obtain all required permits and accept responsibility for commissioning, testing and final verification where indicated in contract documents, except as otherwise expressly assigned in writing.

Each party shall comply with applicable privacy and protected health information obligations in connection with the review process. Design Firm shall implement reasonable safeguards to protect any patient-identifiable information accessed for design purposes. The parties agree to cooperate in correction of design deficiencies identified during construction and commissioning and to indemnify one another only to the extent required by their underlying contractual agreements.

Acknowledgments

The undersigned representatives certify that they are authorized to act on behalf of the named parties, that the information contained in this form and the attached submission package is accurate to the best of their knowledge, and that signing this document constitutes formal acceptance of the review determination and the Conditions of Approval stated above.

What the Healthcare Site Design Approval Is

The Healthcare Site Design Approval is a structured document package used to record and authorize proposed facility layout, systems, and operational details for a healthcare location. It combines schematic drawings, floor plans, mechanical/electrical/ plumbing (MEP) specifications, infection-control measures, accessibility notes, and an approval block for stakeholders and authorities having jurisdiction. The package establishes a clear, auditable decision record that guides construction, remodels, licensing inspections, and accreditation reviews while documenting the responsible parties and effective dates of approval.

Why a Formal Approval Package Matters

A formal Healthcare Site Design Approval centralizes technical details, aligns clinical and operational requirements, and creates an auditable record that supports compliance, permits, and accreditation. Properly completed approvals reduce rework, clarify responsibilities, and document the basis for construction and licensing decisions under applicable laws and standards.

Why a Formal Approval Package Matters

Who Typically Prepares and Signs This Approval

Multiple stakeholders participate in site design approvals to ensure clinical, safety, and regulatory requirements are addressed.

  • Healthcare facility managers and operations directors who verify clinical workflows and resource placement.
  • Architects and design firms responsible for plans, code compliance, and technical specifications.
  • Compliance officers, infection control leads, and facility engineers who confirm standards and documentation.

Each signer should have authority for their area; final sign-off usually rests with the facility owner or authorized representative.

Step-by-step: Completing a Healthcare Site Design Approval

Follow this sequence to prepare and obtain approvals efficiently.

  • 01
Gather Documents: Collect drawings, MEP specs, and program requirements.
  • 02
Assign Reviewers: List clinical, safety, and engineering approvers in order.
  • 03
Submit Package: Provide files to AHJ, accreditor, and internal reviewers.
  • 04
Record Approval: Capture signatures, dates, and version numbers.

Configuring an online approval workflow

Typical online workflows automate routing, authentication, and storage; configure these settings before sending.

Field Configuration
Authentication Email link, SMS code, or two-factor for higher assurance
Notifications Auto-reminders at set intervals until signing completes
Routing Order Sequential or parallel signer order set by role
Storage Location Designate secure cloud folder or records repository

Where to submit or distribute the approved package

After sign-off, route copies to authorities, internal teams, and vendors to begin implementation.

  • Authority Having Jurisdiction: Submit final package for permitting or licensing review.
  • Internal Records: Store signed package with facilities and compliance teams.
  • Contractors: Provide approved drawings and specs to construction partners.
  • Accreditors: Share package for accreditation or inspection evidence.

Technical considerations for digital submission

Ensure chosen systems support secure file formats, signer authentication, and retention requirements before e‑submission.

  • File formats: PDF, DOCX, and CAD PDFs are commonly accepted
  • Integrations: Connectors with document management and EHR systems recommended
  • Authentication: Email, SMS, or multi-factor signer verification

Confirm the platform provides an audit trail, tamper-evident storage, and the ability to export signed records for regulatory audits.

Core elements to include in a professional approval package

A complete package should present technical, clinical, and regulatory information in a clear, organized way to expedite review.

Scope Diagram

Concise plan showing boundaries, room adjacencies, and program elements so reviewers understand the project footprint and interfaces.

Floor Plans

Scaled drawings with room names, sizes, door swings, and clearances for clinical workflows and code compliance checks.

MEP Specifications

Mechanical, electrical, and plumbing system details including HVAC zoning, electrical loads, and medical gas requirements where applicable.

Accessibility Notes

Information on ADA features, clearances, and accessible routes to demonstrate compliance with accessibility standards and local codes.

Infection Control

Material selections, ventilation strategy, and cleaning access points described to meet clinical infection-prevention requirements.

Approval Signatures

Designated signature block for stakeholders, dated and versioned to create a definitive, auditable approval record.

Essential data elements to capture

Project ID: Unique identifier
Facility Name: Legal entity
Address: Street, city, state
Approver Names: Printed and role
Sign Date: MM/DD/YYYY format
Version Number: Revision tracking

Practical tips for accurate and efficient approvals

Adopt consistent practices to reduce review cycles and maintain compliance.

Maintain a single source of truth
Use version-controlled documents and label revisions clearly; ensure reviewers access the latest set to prevent conflicting instructions during construction.
Define reviewer responsibilities
List required approvers and their scope (clinical, engineering, compliance) so each party knows what to verify before signing.
Use clear schedule notes
Attach a concise implementation schedule and note any phasing to avoid misinterpretation of which areas are approved for immediate work.
Capture rationale for exceptions
If deviations from standard practice are approved, record the reason, duration, and risk mitigation so future auditors and owners understand the basis.

Typical timelines and processing expectations

Processing times vary by jurisdiction and project scope; plan submissions with local review cycles in mind.

Local AHJ Review:

Commonly 30–90 days, depending on workload and complexity.

Internal Review Cycle:

Allow 7–21 days for multi-discipline stakeholder responses.

Revisions and Resubmission:

Expect 2–4 weeks when plan corrections are required.

Permit Issuance:

Timing depends on AHJ workload and outstanding conditions.

Construction Start:

Do not begin until required approvals and permits are documented.

Key risks and potential consequences of incomplete approvals

Regulatory Fines: Civil penalties possible
Construction Stop: Work may be halted
Insurance Denial: Claims risk if unapproved work
Patient Safety: Inadequate design increases hazards
Contract Disputes: Change orders and claims
Retention Failure: Missing records hinder audits

How organizations use approved site design records in practice

Real-world examples show common workflows and practical outcomes for approved packages.

Fertility Centers of Illinois — John Butler

The center centralized site plans and clinical flow documentation to streamline reviews.

  • Approval tracking shortened review handoffs.
  • The auditable record supported accreditation submissions and maintained required HIPAA safeguards while reducing paper routing and lost revisions.

Martin Properties — Tim Martin

A development firm used signed approvals to coordinate contractors and vendors across sites.

  • Version control prevented rework.
  • Clear sign-off blocks reduced change orders, improved contractor accountability, and preserved a single master set for permitting and occupancy inspections.
