Shared Medical Appointment (SMA) Consent and Authorization

Purpose: I understand the Shared Medical Appointment (SMA) is a group-format clinical encounter in which multiple patients are seen together for education, assessment, and coordinated care. My primary reason for attending this SMA is:

Consent to Participate: By checking below I voluntarily consent to participate in the SMA. I acknowledge that the SMA involves group discussion of health information and interaction with other patients and clinical staff.

I consent to participate in the Shared Medical Appointment and to group-based care and education.

Confidentiality and Limits: The clinic and providers will make reasonable efforts to preserve the confidentiality of my medical information disclosed during the SMA. However, confidentiality cannot be absolutely guaranteed in a group setting. I understand the following exceptions permit disclosure of information without my further authorization: 1) suspected child, elder, or dependent adult abuse or neglect; 2) threats of harm to self or others; 3) disclosures required by court order or other legal process; and 4) as required by public health reporting laws. I agree that if I disclose personally identifying information about other participants outside the SMA, such disclosure may breach confidentiality obligations and may subject me to removal from the SMA.

Authorization to Share Clinical Information: I authorize the providers facilitating this SMA to share my relevant medical history, treatment plan, and limited clinical data with other SMA participants and with the multidisciplinary clinical team for the purposes of coordinated care. This authorization does not authorize release of psychotherapy notes or unrelated sensitive information, unless separately specified below.

I consent to audio/video recording of the SMA session for clinical documentation and team review only. Recordings will be stored in secure clinical records and will not be used for public distribution.

Risks and Benefits: Potential benefits include improved access to care, peer support, and more comprehensive education. Risks include inadvertent disclosure of personal health information and reduced privacy compared to individual visits. I have had the opportunity to ask questions about risks and benefits and my questions have been answered to my satisfaction.

Voluntary Nature and Right to Withdraw: Participation is voluntary. I may refuse to participate in or may withdraw from the SMA at any time without penalty and without jeopardizing my access to other clinic services. To withdraw this authorization for future sharing, I must provide written notice to the clinic. Withdrawal will not affect disclosures made prior to receipt of written withdrawal.

If no date is provided, this authorization will expire one year from the date signed.

HIPAA Privacy Acknowledgment

I acknowledge that I have been offered or received the clinic's Notice of Privacy Practices describing how my protected health information may be used and disclosed. I understand that information discussed in the SMA may be included in my medical record and accessed by the care team in accordance with applicable privacy rules.

I acknowledge receipt of the Privacy Practices and consent to uses and disclosures described above.

Patient Certification and Agreement

By signing below I certify that the information provided on this form is accurate to the best of my knowledge. I understand that providing false or misleading information may affect my care. I authorize clinicians to document and use information as described in this SMA authorization and release the clinic from liability for disclosures made in good faith consistent with this authorization and applicable law.

What the Healthcare SMA Document Is

The Healthcare SMA Document is a written services-management agreement commonly used between healthcare providers, clinics, and vendor partners to define scope, responsibilities, data handling, and payment terms. It organizes operational obligations, privacy and security expectations, performance metrics, and billing rules into a single contract intended for recurring or ongoing services. For healthcare contexts, the document typically includes provisions addressing protected health information (PHI), business associate relationships, compliance with HIPAA privacy and security standards, and procedures for incident reporting and audit access.

Why the Healthcare SMA Document Matters

A clear Healthcare SMA Document reduces operational ambiguity, protects patient data, and allocates compliance responsibilities between parties. It documents obligations for PHI handling, sets measurable service levels, and clarifies payment and termination mechanics to limit downstream disputes.

Why the Healthcare SMA Document Matters

Typical Users and When They Sign

Teams across provider organizations, vendor operations, and compliance departments prepare and sign the Healthcare SMA Document when establishing service relationships or renewing managed services agreements.

  • Healthcare Providers — Hospitals, clinics, and medical groups that outsource administrative or technical services to third parties.
  • Vendors / MSPs — Companies delivering staffing, IT, billing, or clinical-support services under recurring arrangements.
  • Compliance Teams — Legal counsel, privacy officers, or contract managers who validate HIPAA, BAA, and security provisions.

Signatures usually come from authorized contracting officers, clinical leaders, or delegated executives; legal and compliance review is common before execution.

Representative Signatories and Their Roles

Clinical Director

Clinical leaders usually approve scope of clinical services and quality metrics. They ensure clinical workflows and patient-safety obligations are accurately reflected and escalate clinical incidents to contract governance teams for remediation and reporting.

Contracting Officer

A contracting officer or authorized executive signs financial and legal terms. They confirm authority to bind the organization, negotiate indemnities and liability caps, and coordinate execution with privacy and procurement teams.

Step-by-Step: Completing the Healthcare SMA Document

Follow these sequential steps to prepare, review, and execute the Healthcare SMA Document with minimal revisions and regulatory risk.

  • 01
Prepare draft: Populate parties, scope, and effective date.
  • 02
Attach BAA: Include a HIPAA BAA if PHI will be exchanged.
  • 03
Obtain reviews: Route to legal and privacy for review.
  • 04
Execute: Collect authorized signatures and store executed copy.

Typical eSubmission Flow for the Document

A standard electronic workflow moves the draft from preparation through signature, evidence capture, and secure storage.

  • Upload: Import agreement PDF or DOCX into the signing platform.
  • Place fields: Add signature, date, and initial fields for each signer.
  • Authenticate: Choose signer authentication methods as required.
  • Archive: Save signed PDF and retention metadata securely.

Recommended Online Workflow Settings

Configure e-sign workflows to reduce friction while meeting HIPAA and audit requirements.

Field Configuration
Signer Order Sequential order with delegated signing options
Authentication Email plus SMS code or organization SSO
Attachments Require BAA and supporting exhibits before signing
Reminders Automated reminders with escalation after 7 days

Technical and Integration Considerations

Choose a platform that supports secure storage, audit logging, and HIPAA-compliant workflows when PHI is involved.

  • File formats: PDF, DOCX accepted
  • Integrations: Works with EHR and document repositories
  • Authentication: Supports SMS and SSO

Confirm integration options for your EHR, archive system, and identity provider to ensure end-to-end security and reliable recordkeeping across operational systems.

Essential Security and Compliance Elements

PHI Protections: Encryption and access controls
BAA Requirement: Business Associate Agreement
Audit Trail: Immutable signing history
Authentication: Multi-factor options
Encryption: TLS 1.2/1.3 and AES-256
Compliance: HIPAA, SOC 2, ISO 27001

Common Preparation Pitfalls to Avoid

  • Using informal or inconsistent party names that do not match tax or corporate records, which can delay signature authority or payment processing.
  • Failing to attach a required Business Associate Agreement when PHI is involved, exposing parties to HIPAA compliance gaps and potential fines.
  • Overlooking explicit data-handling procedures for subcontractors and vendors, which creates ambiguity about breach responsibilities and reporting timelines.
  • Leaving payment terms vague or omitting invoicing schedules, which often results in disputes and delayed reimbursements.

Key Risks and Potential Penalties

HIPAA Fines: Civil and criminal penalties
Contract Breach: Monetary damages and injunctive relief
Data Exposure: Notification costs and remediation
I-9 Violations: Paperwork fines apply
Tax Reporting: Backup withholding triggers
Operational Delay: Service interruptions and revenue loss
