Release and Billing Authorization

By checking the box below, the Patient authorizes the Facility to bill the Sponsor for covered services and to disclose the minimum necessary protected health information to the Sponsor and its administrators for the purpose of verification, eligibility determination, claims processing, and payment.

The Patient acknowledges that Sponsor may require additional documentation prior to payment. If the Sponsor denies payment or fails to pay amounts due, Patient remains responsible for unpaid balances, unless the Sponsor has explicitly agreed in writing to assume liability for all charges.

Patient Responsibilities and Acknowledgements

Patient acknowledges and agrees to the following:

  • Patient must provide accurate Sponsor information and update the Facility of any changes.
  • Patient remains responsible for any portion of charges not covered by the Sponsor, including co-payments, deductibles, non-covered services, and any amounts denied by the Sponsor.
  • Patient consents to the Facility communicating with the Sponsor and its agents as necessary to obtain payment.
  • Patient may revoke this authorization in writing; revocation will not affect disclosures already made in reliance on this authorization prior to receipt of the revocation.

HIPAA / Privacy Acknowledgement

Patient acknowledges receipt of the Facility's notice of privacy practices and understands that the Facility may disclose protected health information to the Sponsor only as authorized above. Patient understands that the Facility will limit disclosures to the minimum necessary information to accomplish the intended purpose.

Representations and Legal Terms

Patient represents that the information provided in this form is true and correct. Patient understands that any false statement or omission of material fact may be grounds for denial of sponsorship coverage and may render Patient financially responsible for services provided. Patient further authorizes the Facility to act on the Sponsor information provided and to pursue any remedies available to collect unpaid balances if the Sponsor does not pay as represented.

This Acknowledgement does not create any contract between the Facility and the Sponsor. The Facility's acceptance of Sponsor information for billing purposes does not guarantee payment by the Sponsor. The Facility reserves the right to seek payment from the Patient if Sponsor payment is not received.

Patient may revoke this authorization at any time by providing written notice to the Facility; such revocation will not affect disclosures already made in reliance on this authorization prior to receipt of the revocation. Upon expiration or revocation, any further billing to the Sponsor will cease, and Patient will be responsible for future charges.

What the Healthcare Sponsorship Acknowledgement Is

A Healthcare Sponsorship Acknowledgement is a written record documenting that an individual or organization agrees to sponsor or support a patient, visitor, or participant for access to healthcare services, visits, or related programs. The form establishes the sponsor's identity, relationship to the subject, scope of financial or logistical responsibility, and any privacy or data-sharing consents required for care coordination. It is used by hospitals, clinics, research sites, and community health programs to capture clear, auditable acceptance of sponsorship obligations and to reduce administrative disputes.

Why this Acknowledgement Matters

The acknowledgement clarifies responsibilities, documents consent for information sharing, and creates an audit trail that supports billing, access control, and compliance with healthcare rules like HIPAA.

Why this Acknowledgement Matters

Who Typically Completes This Form

The Healthcare Sponsorship Acknowledgement is completed by individuals or organizations providing financial, logistical, or custodial support for a patient or program participant.

  • Family members or guardians providing guaranteed payment or visitation support
  • Nonprofit or community organizations sponsoring program participation
  • Employers or relocation services covering employee medical costs or transport

Organizations use the form to standardize intake and reduce disputes about who is responsible for payment, transportation, or privacy consents.

Use platforms that support secure PDF, audit trails, and appropriate signer authentication for healthcare data.

  • File formats: PDF, DOCX accepted; use flattened PDF for archival
  • Integrations: Connects with EHRs, CRM, and cloud storage systems
  • Authentication: Support email, SMS, or advanced methods for PHI

Ensure the chosen platform can produce an audit trail, export signed copies, and support required retention and access controls.

Required information elements at a glance

Sponsor identity: Full legal name
Sponsor contact: Street address and phone
Recipient identity: Patient full name
Sponsorship scope: Detailed responsibilities

Key risks and legal consequences

Unauthorized disclosure: Potential breach and penalties
False sponsorship: Fraud exposure and civil liability
Retention failure: Noncompliance with record rules
Paperwork delays: Service or billing interruptions

Common preparation errors to avoid

  • Using initials instead of full printed names, causing signer attribution disputes.
  • Leaving sponsorship scope vague, which creates disagreement over covered costs or services.
  • Failing to attach supporting authorization, delaying verification and billing processes.
  • Not capturing a dated signature, which obscures when obligations and consents began.

Practical tips for accurate completion

Adopt consistent internal rules and templates so every acknowledgement includes necessary verification, consent, and scope details.

Use standardized templates
A consistent form reduces missing fields and speeds matching to medical records and billing systems.

Capture consent clearly
Include explicit language for information sharing and data use to meet HIPAA and internal privacy policies.
Record retention plan
Document where signed forms are stored, who can access them, and retention periods to support audits.
