Telepsychology Services: Description and Consent

Telepsychology involves the provision of psychological services using interactive audio, video, or messaging technologies. The nature of telepsychology includes evaluation, psychotherapy, assessment, and coordination of care as indicated by the treating clinician.

I understand that telepsychology differs from in-person services and that there are benefits and risks, including but not limited to: interruptions in service, inadvertent disclosure of information, limitations on physical examination, and limitations of crisis response across distances.

By checking the boxes below, I acknowledge understanding and consent:

Confidentiality and Limits

All the protections of confidentiality that apply to in-person services also apply to telepsychology services, with the following exceptions and clarifications: technical failures may compromise privacy; information may be visible to others if sessions are conducted in a non-private location; the clinician may need to communicate electronically with other health professionals for coordination of care.

I understand there are legally mandated exceptions to confidentiality, including but not limited to: suspected child abuse or elder abuse, imminent risk of harm to self or others, and court orders. The clinician will make reasonable efforts to inform me when disclosure is required or permitted.

Emergency Procedures and Local Resources

Telepsychology is not appropriate for psychiatric emergencies. If I am experiencing an emergency or I am in imminent danger, I will use local emergency services. To assist with emergency response, I provide the following local information:

Fees, Scheduling, and Cancellation

Fees for telepsychology sessions are the same as for in-person sessions unless otherwise stated. I am responsible for charges not covered by insurance. Cancellation policy applies to telepsychology appointments; fees may be charged for missed or late-cancelled sessions as permitted by law and provider policy.

HIPAA / Privacy Acknowledgment

I acknowledge that I have been offered or provided with an applicable Notice of Privacy Practices that describes how my protected health information is used and disclosed, including use in telehealth. Electronic communications may carry additional privacy risks which have been described to me above.

Authorization and Revocation

I authorize the use of telepsychology as described above. I understand this authorization will remain in effect until the expiration date I specify below or until I revoke it in writing. Revocation will not apply to information already disclosed in reliance on this authorization.

Consent Certification

By signing below I certify that I have read and understand this Telepsychology Consent Form, that my questions have been answered, and that I agree to the terms described herein. I understand that I may withdraw consent at any time by providing written notice to my provider.

What the Healthcare Telepsychology Consent Form Covers

The Healthcare Telepsychology Consent Form documents a patient's informed agreement to receive psychological services remotely. It explains the telehealth modality, technical requirements, confidentiality limits, security measures, emergency procedures, and billing or insurance implications. The form records contact and emergency information, consent to electronic communication or recording where applicable, and authorization to share records with other providers when needed. Written consent prior to the first telepsychology session supports clinical clarity, risk management, and legal compliance across U.S. jurisdictions. It can be signed electronically under ESIGN and state e-signature laws.

Why a Formal Telepsychology Consent Matters

Using a formal Healthcare Telepsychology Consent Form documents patient understanding of telehealth risks and benefits, supports clinical documentation, and helps satisfy ESIGN and state electronic transaction rules such as UETA. It clarifies HIPAA privacy expectations and emergency procedures before care begins.

Why a Formal Telepsychology Consent Matters

Who Completes and Relies on This Form

Typical users include clinicians, patients, and organizations that offer remote psychological services and telehealth program administrators.

  • Licensed psychologists and therapists managing remote treatment across platforms and maintaining documentation and safety plans.
  • Patients or guardians giving informed consent for telepsychology sessions and electronic communications.
  • Health systems, clinics, and private practices standardizing telehealth consent workflows.

Organizations should integrate the form into intake and electronic health record workflows and review periodically.

  • Using insecure platforms or failing to document technical failures and security incidents increases HIPAA breach risk and liability.
  • Vague or missing emergency plans leave clinicians uncertain how to respond to crises and may violate standard of care.
  • Allowing unsigned or improperly witnessed consents, especially for minors or guardians, can lead to denial of insurance reimbursement or court challenges.

Potential Consequences of Incomplete or Improper Consent

HIPAA Fines: Civil penalties and corrective action plans.
Invalid Consent: Treatment authorization may be void.
Malpractice Claim: Increased liability and potential lawsuits.
Insurance Denial: Claims may be denied or delayed.
