Consent to Treatment: I, the undersigned patient or authorized representative, consent to routine and emergency medical care and procedures as deemed necessary by the treating clinician. I understand that no guarantee has been made as to the results of any treatment. Dispute Resolution and Legal Terms

Governing Law and Dispute Resolution: This Agreement shall be governed by the laws of the state in which services are rendered. Any dispute arising from or related to this Agreement, including claims of malpractice or billing disputes, shall be resolved in the state or federal courts located in the jurisdiction of the treating facility unless the parties mutually agree to arbitration. If arbitration is elected, the arbitrator's decision shall be final and binding.
