I understand I may withdraw from the program at any time by providing written notice to program staff and that withdrawal does not relieve me of financial obligations for services already rendered in accordance with the Payment and Cancellation section below.

Payment, Billing, and Cancellation

I agree to pay program fees as outlined in my enrollment agreement. I authorize the program to bill my insurance when appropriate and to collect any co-payments, co-insurance, or amounts not covered by insurance directly from me. I understand that missed appointments and late cancellations may be subject to fees and that refunds for program fees will be issued only in accordance with the program's published cancellation and refund policy. Failure to pay outstanding balances may result in suspension or termination of program services.

Consent to Treatment & Authorizations

By signing this form I provide informed consent for clinicians and authorized program staff to perform assessments, provide treatment modalities that are part of the program, and coordinate care with my other healthcare providers. I authorize release of relevant health information to other clinicians involved in my care for purposes of treatment, billing, and case coordination.

I authorize the program to use and disclose my protected health information (PHI) as necessary to provide care, to consult with other providers, for quality improvement, and for billing and payment purposes. This authorization includes communication by phone, text, and secure electronic methods as required for care coordination.

I understand I may revoke this authorization at any time by delivering written notice, except to the extent that the program has already acted in reliance on this authorization. Revocation does not affect disclosures made in reliance on the authorization prior to revocation.

Media Release & Communications

I consent to the program taking photographs, audio, or video recordings of my participation for treatment documentation and program quality assurance. I further consent to the use of de-identified images or recordings for education or program improvement.
