Trainee acknowledges that access to Protected Health Information (PHI) may be necessary for clinical training. Trainee will comply with all applicable privacy and security laws and Facility policies. Trainee agrees not to access, use, disclose, or record PHI except as expressly permitted for the purposes of training and patient care under supervision. Unauthorized use or disclosure of PHI is grounds for immediate termination and may result in civil and criminal liability.

Trainee acknowledges receipt of the Facility's Privacy and Security rules relevant to trainees and agrees to complete privacy training prior to unsupervised access to clinical systems. Trainee initial to confirm understanding:

Liability, Insurance and Indemnification

The Facility maintains liability coverage for Facility operations. To the fullest extent permitted by law, the Trainee agrees to indemnify and hold harmless the Facility from third-party claims arising from Trainee's negligent acts or omissions during training.

Materials, Records and Intellectual Property

Training materials provided by the Facility remain the Facility's property unless expressly stated otherwise. Trainee may not reproduce or distribute materials containing PHI. Evaluations, competency checklists, and training records created during the term are property of the Facility and may be retained in trainee personnel or program files.

Compensation and Expenses

Compensation: . Unless otherwise agreed in writing, Trainee shall bear personal expenses including travel, housing, and required immunizations.

Termination

Either party may terminate this Agreement upon written notice to the other party for material breach, failure to meet health or safety requirements, criminal conduct, or behavior inconsistent with professional standards.
