Client agrees to pay all charges for professional services and materials at the time services are rendered unless other written arrangements are made in advance. Provider may require a deposit for surgical or specialty procedures. If payment is not received within 30 days, a finance charge and collection costs may be applied.

6. Consent for Anesthesia, Surgery and Procedures

The Client authorizes the Provider to perform the services described above and such additional diagnostics or treatments as are necessary for the health and welfare of the patient during the course of treatment. The Client acknowledges that anesthesia and surgical procedures carry inherent risks including, but not limited to, adverse reactions to drugs, hemorrhage, infection, organ dysfunction, neurologic injury, and death. The Client affirms that the risks, benefits, and alternatives have been explained and accepts these risks.

The Client authorizes routine monitoring, use of prescribed analgesia and antimicrobials, and minor adjustments intraoperatively deemed necessary by the attending veterinarian. The Provider will attempt to contact the Client for consent for significant deviations from the planned procedure; if the Client cannot be reached and immediate treatment is required to prevent death or severe harm, the Provider is authorized to proceed.

7. Emergency Treatment Authorization

In the event of an emergency, the Provider is authorized to provide stabilization and emergency treatment. The Client (or emergency contact) will be notified as soon as practicable. The Client understands that emergency care may require immediate actions without prior consent to prevent serious injury or death.

8. Release of Records and Confidentiality

Client authorizes the Provider to release medical records and billing information to third parties only with Client authorization, except as required by law. The Client may request copies of the patient record; Provider may charge a reasonable copying fee.

9. Limitation of Liability and Indemnification

Provider will exercise reasonable care and professional judgment in performing services. Except as otherwise provided by applicable law, Provider's liability for claims arising under this Agreement shall be limited to direct damages and shall not include punitive, consequential, or special damages. Client agrees to indemnify and hold Provider harmless from any claims, losses, or expenses arising from Client's failure to disclose material medical history or adhere to post-treatment instructions.

10. Abandonment and Termination

If Client fails to collect patient within 7 days after written notice that patient is ready for discharge, Provider may consider the patient abandoned and may proceed with disposition in accordance with applicable law. Provider may terminate services for nonpayment or breach of this Agreement upon written notice.
