I understand and acknowledge that veterinary medicine is not an exact science and that no guarantees have been made to me regarding the results of examinations or treatments.

I consent to the administration of medications, vaccines, anesthetics, and analgesics as needed. I understand that all procedures carry inherent risks including but not limited to adverse reaction to anesthesia or medications, infection, bleeding, and postoperative complications. I consent to diagnostic procedures as recommended, including radiographs, bloodwork, cytology, and biopsy.

I acknowledge that the veterinarian will exercise reasonable professional judgment to manage complications. I authorize the clinic to perform lifesaving emergency procedures if my animal’s life is at risk and I cannot be reached at the contact numbers provided. I understand that such emergency interventions may incur additional charges for which I am responsible.

Euthanasia: I understand that euthanasia is a distinct medical and ethical decision and will not be performed without my explicit, separate consent except in circumstances where it is necessary to alleviate extreme suffering and the owner/agent cannot be reached and delay would result in unjustified suffering. Financial Responsibility: I agree to pay all charges in full at the time services are rendered unless other arrangements have been expressly approved in writing by the clinic. I understand that in the event of nonpayment the clinic may pursue collection and I will be responsible for collection costs and reasonable attorney fees.
