What the Housing Client Intake Agreement Is

A Housing Client Intake Agreement is a standardized form used by housing providers, case managers, landlords, and assistance programs to collect applicant information, consent to screenings, and record eligibility for housing services. It gathers identifying details, income and household data, disclosures, and consent for background or credit checks. The agreement creates a clear, reproducible record of the applicant’s representations and authorizations, and it establishes the provider’s scope for assessing eligibility, sharing information with third parties, and documenting next steps in housing placement or assistance processes.

Why a Clear Intake Agreement Matters

A well‑structured Housing Client Intake Agreement reduces processing delays, improves documentation accuracy, and protects both client and provider by recording consent and disclosures in one retrievable record.

Why a Clear Intake Agreement Matters

Who Typically Completes This Form

The Housing Client Intake Agreement is used by a range of housing stakeholders to start an applicant file and trigger eligibility checks.

  • Public housing agencies and housing authorities that screen applicants for waitlists and vouchers.
  • Nonprofit shelters and service providers managing intake for emergency or transitional housing.
  • Property managers and landlords who collect tenant background and household information.

Use the form at first contact, during intake appointments, or as a required step before eligibility determinations or lease offers.
