What the Identity Verification Premium Form Is

The Identity Verification Premium Form is a structured document used to collect verified identity data, supporting identification documents, and explicit attestations when higher assurance is required. It combines personal details, government-issued ID fields, optional biometric or credential-analysis outputs, and signer declarations so organizations can meet internal risk thresholds and audit requirements. Designed for electronic completion and secure submission, the form supports remote online notarization, layered authentication, and an audit trail to align with U.S. e-signature law and record-retention practices for regulated workflows.

Why organizations use this premium identity form

The form raises identity-assurance levels for onboarding, high-risk transactions, and regulatory compliance by standardizing evidence capture, preserving audit metadata, and enabling secure electronic workflows that meet ESIGN and UETA legal tests.

Practical tips to improve accuracy and speed

Adopt these practices to reduce errors, speed verification, and maintain legal defensibility for signed identity records.

Request Exact Name Matches
Require the signer to enter their name exactly as shown on their primary ID; match logic should flag small variations for manual review rather than auto-fail.
Use High-Quality Image Capture
Provide in-form guidance for ID photo clarity and lighting; clear images reduce false negatives during credential analysis and manual review workload.
Automate Audit Capture
Configure the platform to record IP, device data, timestamps, and verification outputs automatically so the record is complete and exportable for audits.
Plan for Exceptions
Define a manual review and escalation workflow for mismatches, expired IDs, or flagged biometric results to avoid process bottlenecks.
