SOLE CUSTODY AND VISITATION SCHEDULE 1. Sole Custody . Sole Custody of the following minor children shall be with ____________________________ hereafter referred to as ______________________________. Sole Custody means both legal and physical custody of the children. The parent awarded sole custody shall decide all matters customarily decided by parents on behalf of minor children. 2. Visit ation . _________________________________ hereafter referred to as _______________________________ shall have reasonable visitation. The parties may agree to vary the following visitation schedule. However, if they fail to agree, the following visitation schedule must be followed and it shall take effect upon 5 days notice in writing by either party. _______________________ shall have visitation on the following holidays and special days: Even Numbered Years Martin Luther King Day Easter Sunday & Monday Labor Day Veteran's Day Christmas Eve Day Christmas Vacation from January 1 until the evening before resumption of school Birthday of _____________________________ (On the birthdays of each child all children shall be in the physical custody of the parent celebrating the birthday of the child in question. Note: If more than one child, consider alternating years between parents.) Odd Numbered Years Lincoln's Birthday Good Friday & Easter Saturday Memorial Day Columbus Day Thanksgiving Thursday & Friday Chris tmas Day & the next 6 days to Dec. 31 evening On holidays, birthdays, Mother's Day and Father's Day visitation with the children shall being at ______A.M. and end at ______P.M. During Easter, Christmas Vacation and Thanksgiving, visitation shall begin a t _______A.M. of the first day and end at _______P.M. of the last day. If a holiday or special day precedes or follows weekend visitation, ________________________ may keep the children in their care overnight between the holiday or special day and the we ekend visitation period. Holidays and special days shall prevail over weekends and weekday visitation or physical custody, and no make -up days shall be allowed. On Father's Day and Mother's Day and on the birthday of each parent all of the children shall be with the parent celebrating the birthday or Mother’s Day or Father’s Day and those days shall prevail over all other days. 3. Transportation. ________________________ shall transport the children to the residence of _________________________ at the beg inning time for all visitation, and ________________________ shall return the children to the residence of ________________________ at the termination of visitation. 4. Summer Vacation. During the children's Summer vacation ________________________ shall have visitation from _______ P.M. on the 7th day following the last school day of Spring Semester to _______ P.M. on the 7th day prior to first day of school in the Fall Semester. While the children are with ________________________ during said Summer va cation, ________________________ shall be entitled to alternating weekends of physical custody from _____P.M. Friday to _____P.M. Sunday beginning with the third weekend following the last day of school. Each parent shall give the other parent written not ice of the consecutive days that they desire to have physical custody of the children for the purpose of being with the children during their work vacation. The written notice shall be received by the other parent no later than May 1 of each year. In o dd numbered years the selection of ________________________ shall prevail over all visitation or physical custody times of the other parent. In even numbered years the selection of ________________________ shall prevail. 5. Telephone Communication and Pho tographs . When the children are in the physical custody of the custodial parent, and when they are visiting the non -custodial parent, the other parent shall be allowed to communicate with the children by telephone and the children shall be allowed to call and receive phone calls at all reasonable times. The parent making the call to the children is to pay for the call. If the children call, they shall call collect. The children must be allowed to display photos of both parents in the homes of both paren ts and shall be encouraged to do so. 6. Removal of Children from the State of Illinois . Neither party may remove the children from the State of Illinois without permission of the Court, except that either party may do so for a period of time not more tha n 16 consecutive days, consistent with the paragraph relating to Summer vacation. 7. Parental Notice . Each parent shall keep the other informed as to the exact place where each of them resides, the phone numbers of their homes and places of employment, a nd if either parent travels out -of-town for any period of more than (3) days then such person shall notify the other of his or her destination and shall provide a phone number where he or she can be reached. 8. Child Endangerment Prohibition . Both parent s shall at all times, when the children are under their physical care, refrain from placing the children in an environment or exposing the children to activities that may endanger the children's physical, mental, emotional or moral well - being. Both parent s shall avoid controlled substances or excessive use of alcoholic beverages or prescription medication when the minor children are in their care. 9. Emergencies and Medical Care . In case of emergency the parent who has the children in their care shall tak e whatever emergency action is reasonably necessary to meet the health care or other emergency need. As soon as possible, the parent making such an emergency decision will advise the other parent of the same.________________________ shall provide the othe r parent with any medications which the child is taking at the time of transfer for visitation. 10. Access to Records and Information. (Custodial Parent) shall sign the necessary releases and forms so as to allow ________________________ access to school, medical, dental, ocular and psychological or mental health records of the minor children.
