RECITALS

WHEREAS, the Applicant is a noncitizen seeking issuance of a travel document for international travel, reentry to the United States, or humanitarian reasons; and

WHEREAS, the Applicant submits this Application for Travel Document to request one of the travel documents enumerated below and to certify the truthfulness of the statements and evidence provided; and

WHEREAS, the issuing authority requires the Applicant to provide identifying information, documentary proof of eligibility, and an acknowledgement of legal consequences for false statements;

NOW, THEREFORE, in consideration of the submissions made by Applicant and for good and valuable consideration, the Applicant applies for the requested travel document under the terms and conditions set forth herein.

I understand that issuance of a travel document does not guarantee admission to the United States.

6. NOTICES, AMENDMENTS, AND WAIVERS

Notices regarding this application will be deemed effective when mailed to the U.S. mailing address provided herein or delivered in accordance with the rules of the issuing authority. Any amendment to this application must be submitted in writing and signed by the Applicant or authorized representative.

No waiver of any provision of this application will be effective unless in writing and signed by the party waiving enforcement. Failure to enforce any provision shall not constitute a waiver of future enforcement.

7. GOVERNING LAW; ENTIRE AGREEMENT; SEVERABILITY

This application and any determinations made pursuant to it shall be governed by and construed in accordance with federal immigration law and applicable federal regulations. State law shall not alter or control the substantive standards applicable to adjudication.

This document, together with any attachments and declarations submitted in support, constitutes the entire application package submitted by the Applicant. Any prior or contemporaneous oral or written statements that are not incorporated herein are superseded.

If any provision of this application is deemed invalid or unenforceable, the remaining provisions shall remain in full force and effect.
