What an Index Request Form Is and when it’s used

An Index Request Form is a standardized submission used to request a new or corrected index entry with a recorder, title company, or document management system. It ties a document to the public record or an internal index by providing identifying data, jurisdiction, and filing instructions. Typical uses include property recordings, lien indexing, document retrieval indexing, and administrative record corrections. The form organizes metadata so clerks or indexing systems can locate, classify, and attach the referenced document consistently across databases and registries.

Why accurate index requests matter

A complete Index Request Form reduces processing delays, lowers the chance of misfiled records, and creates a verifiable audit trail that supports future retrieval, title searches, and legal compliance.

Why accurate index requests matter

Who typically completes an Index Request Form

Professionals in records, title, legal, and property administration commonly prepare these forms before filing or recording documents.

  • Title agents and escrow officers who submit recordings for property transfers and need consistent index entries for searches and title reports.
  • Legal and compliance teams who update records, correct indexing errors, or attach court documents to case indices for discovery and auditability.
  • Records managers in government and corporate settings who maintain public registers and internal document indexes for retrieval and statutory reporting.

Clear role assignment speeds approval and minimizes rework when clerks or indexers need additional details.

Step-by-step: completing and submitting an Index Request Form

Follow these sequential steps to prepare an index request that is clear, complete, and ready for recording or internal indexing.

  • 01
    1. Gather details: Collect document metadata and supporting IDs.
  • 02
    2. Complete form: Enter fields exactly and attach the document copy.
  • 03
    3. Verify accuracy: Confirm names, parcel numbers, and jurisdiction.
  • 04
    4. Submit: Send to recorder or indexer using chosen delivery method.

Typical eSubmission workflow for index requests

Electronic submission improves tracking and reduces manual handling; the workflow below outlines common digital steps from preparation to index entry.

  • Upload Document: Attach a PDF or DOCX copy, ensuring the image is legible.
  • Add Fields: Place index metadata fields and required signature fields.
  • Sign Electronically: Signers authenticate and apply signatures consistent with ESIGN/UETA.
  • File or Record: Transmit to county recorder or internal index with payment and metadata.

Configuring an online Index Request workflow

Set up automated routing and verification to reduce manual errors and maintain an auditable process.

Field Configuration
Authentication Email link, SMS code, or knowledge-based verification as required

  • File Formats: PDF, DOCX supported
  • Authentication: Email or SMS codes
  • Integrations: Salesforce, NetSuite, Box

Key elements to include in a professional Index Request Form

A complete form organizes metadata, attachments, authority, and processing instructions so recorders or indexers can act without additional clarification.

Indexing Details

Clear description of the index entry requested, including document title, instrument number, and page or volume references that the recorder will use to place the document in the public record.

Jurisdiction Info

Specify county, state, and any internal department codes so the request is routed to the correct recorder or index team without manual reassignment.

Attachments

Include a legible attachment of the document being indexed plus any supporting forms, maps, or legal descriptions to avoid follow-up requests.

Notarization Details

Indicate whether a notarization or RON is attached, the notary name, commission number, and any audio-video retention notes for remote notarizations.

Audit Trail

Record submission timestamps, signer authentication method, and user IDs so the index action can be audited or defended later.

Receipt and Reference

Provide a reference number or confirmation field that the recorder returns after acceptance to link the physical entry with the electronic request.

Security and compliance features to protect index submissions

Encryption: TLS 1.2/1.3 in transit
Data at Rest: AES-256 encrypted storage
Audit Trail: Complete timestamped event log
Regulatory: ESIGN and UETA compliance
Healthcare: HIPAA support with BAA
Certifications: SOC 2 Type II and ISO 27001

Common penalties and risks from incorrect index requests

Rejection: Missing jurisdictional detail
Recording Delay: Incorrect parcel or ID
Legal Challenge: Incomplete chain of custody
Fee Forfeiture: Late or incorrect payment
Privacy Breach: Unauthorized data exposure
Title Risk: Misindexed documents affecting title searches

Frequent mistakes when preparing an Index Request Form

  • Submitting incomplete property identifiers such as abbreviated parcel numbers or truncated legal descriptions often prevents correct linking and forces resubmission.
  • Using inconsistent names for parties — e.g., abbreviations, initials, or DBA instead of the legal entity — can break automated matching and delay processing.
  • Failing to attach a legible copy of the document or omitting required exhibits causes clerks to reject the request or to request supplemental materials.
  • Ignoring jurisdiction-specific notarization or witness rules leads to rejected filings; confirm county and state requirements before submitting.
