What an Infrastructure Service Request Is

An Infrastructure Service Request is a formal, written request used by organizations to ask for planning, provisioning, maintenance, or modification of infrastructure resources. It standardizes the information needed for approvals, technical evaluation, scheduling, budgeting, and vendor coordination, and it documents scope, location, access, and acceptance criteria for the requested work. The form can be used for physical infrastructure (site utilities, roadwork, building systems) or IT infrastructure (networks, servers, storage, cloud resources), and it supports audit trails for internal controls and vendor accountability.

Why a Standard Request Form Matters

Using a consistent Infrastructure Service Request reduces misunderstandings, speeds approvals, and creates an auditable record of authorization, scope, and cost estimates while supporting compliance with company policy and applicable laws.

Why a Standard Request Form Matters

How to Complete an Infrastructure Service Request

Follow these sequential steps to prepare and submit a complete, approvable request for infrastructure work.

  • 01
Gather details: Collect scope, site plans, and contact information.
  • 02
Estimate costs: Provide budget numbers or attach vendor estimates.
  • 03
Select approvers: Identify budget owner and technical approver.
  • 04
Submit and track: Send via the approved system and monitor status.

Essential Components of a Professional Request

A well-constructed Infrastructure Service Request includes clear scope, contact and location details, technical specifications, timeline requirements, cost estimates, and approvals to minimize back-and-forth and ensure compliant execution.

Scope of Work

Describe tasks in specific, measurable terms, including deliverables, measurable acceptance criteria, and interfaces with existing systems or infrastructure to avoid scope creep and disputes.

Technical Specs

List technical requirements such as capacity, standards, interfaces, or drawings; attach diagrams, vendor part numbers, or configuration files where applicable for precise implementation.

Location & Access

Provide exact site address, building and room, access hours, on-site contact, and any safety or clearance requirements for vendors or technicians.

Timeline & Milestones

Specify required start/completion dates, blackout windows, and milestone acceptance points that will govern scheduling and penalties for missed dates.

Cost Estimate

Include estimated labor, materials, third-party vendor quotes, and contingency amounts so budget owners can make informed approval decisions.

Approvals

Identify required signatories by role and include space for signatures, dates, and any conditional approvals or delegated authorities to ensure valid authorization.

Required Data Elements

Requestor: Full name
Department: Org unit
Service Type: Category
Location: Address details
Schedule: Dates
Approver: Role and name

Where to Send and How Routing Works

Routing depends on organizational policy: requests may go to an internal service desk, facilities team, or procurement; approvals cascade to budget and technical owners before work is scheduled.

  • Submit: Upload form to the service portal.
  • Review: Technical team validates scope.
  • Approve: Budget and manager authorization occurs.
  • Schedule: Work is assigned and scheduled.

How to Configure an Online Request Workflow

When implementing the form in a digital system, configure fields, routing, and notifications to match internal approvals and audit requirements.

Field Configuration
Template Create reusable form with locked sections for standard items.
Conditional Fields Show fields only when relevant to service type.
Authentication Require SSO or multi-factor for approvers.
Routing Automate approver sequence by role.

Distribution and Digital Submission Options

Choose delivery channels and platform integrations that match your IT and procurement ecosystems.

  • Integrations: Salesforce, NetSuite, Microsoft 365
  • File Formats: PDF, DOCX, XLSX
  • Authentication: SSO, SMS, KBA options

Typical Timelines and Processing Expectations

Processing times vary by request complexity and approvals; document realistic lead times to align with procurement and vendor availability.

Initial Acknowledgement:

Within 1–3 business days of submission

Technical Review:

3–7 business days depending on complexity

Procurement Lead Time:

7–30 business days for ordered materials

Scheduling Window:

Based on resource availability and blackout periods

Emergency Response:

Same day to 48 hours for declared emergencies

Common Preparation Mistakes to Avoid

  • Incomplete site or access details causing vendor arrival delays and rescheduling fees.
  • Omitting approvals or budget codes that prevent procurement from issuing purchase orders promptly.
  • Using vague scope language that leads to change orders and cost disputes during execution.
  • Missing safety, permitting, or regulatory notes that result in noncompliance or required rework.

Risks and Potential Consequences of an Incorrect Request

Schedule Delays: Additional downtime
Budget Overruns: Increased costs
Regulatory Noncompliance: Fines or remediation
Safety Incidents: Injury or liability
Contract Disputes: Claims and litigation
Data Exposure: Unauthorized access

Download Options and Supporting Attachments

Provide standard export formats and a list of attachments the request should include to speed review and reduce follow-up questions.

Export Formats

Allow PDF/A for archival, PDF for distribution, and DOCX for editable internal copies; include an audit trail file with signed PDFs.

Site Plans

Attach high-resolution drawings or schematics identifying work areas, utility lines, and access points; label drawing revisions clearly.

Vendor Quotes

Include signed vendor estimates with line-item pricing and delivery lead times to support procurement decisions and approvals.

Permits & Certifications

Attach necessary permits, safety certifications, and insurance certificates required before work can commence to avoid scheduling holds.

How to Update or Revise a Submitted Request

Follow a controlled amendment workflow to avoid unauthorized changes and to preserve the audit trail.

01

Initiate Amendment:

Submit revised request with a clear change summary and reference to the original request ID.
02

Notify Stakeholders:

Alert affected approvers and vendors of the pending change and any impact to schedule or cost.
03

Re-evaluate Scope:

Technical team reviews additional impacts and updates resource estimates.
04

Re-approve:

Obtain approvals from the same roles as the original request or delegated authorities.
05

Document:

Attach change logs and updated drawings to the amended request record.
06

Schedule Update:

Adjust timelines and notify assigned teams of the new schedule.

Notarization and Witness Authentication Steps

When a signed request requires notarization or witnesses, follow these authentication steps to meet state and agency requirements.

01

Prepare Document

Ensure signature blocks and notary sections are present and correctly labeled.

02

Identify Signers

Confirm signers and provide valid government ID for notarization.

03

Choose Notarization

Select in-person or Remote Online Notarization per state rules.

04

Identity Proofing

Complete multi-factor identity checks or credential analysis if required.

05

Witness Presence

Ensure witness count meets state requirements and record witness details.

06

Notary Acknowledgement

Notary completes acknowledgement and signs certificate of notarization.

07

Record AV

For RON, retain audio-video recording per state retention rules.

08

File Copies

Distribute notarized copies to all parties and retain master record.

Require supporting attachments
Mandate schematics, vendor quotes, permits, and site photos where applicable so technical reviewers have the information needed on first pass.
Automate approvals
Map approvers to roles and use automated routing with escalation rules to reduce manual follow-up and reduce turnaround time.
Preserve the audit trail
Ensure signatures, timestamps, and version history are retained in non-editable formats for future verification and compliance checks.
