What the Inquiry LOEX Form Is and when it’s used

The Inquiry LOEX Form is a standardized information-collection template used to record and route formal inquiries, requests for assistance, or project intake related to library instruction, information literacy programs, and academic support services. It captures contact details, affiliation, inquiry type, summary of the request, required deliverables, and preferred response timeframe. The form is suitable for intake by library staff, instructional designers, faculty liaisons, or other campus partners and supports digital submission, secure storage, and tracked responses across departments.

Why a structured Inquiry LOEX Form improves intake and traceability

A consistent inquiry form standardizes information, reduces back-and-forth clarification, and creates an auditable record for follow-up. That record supports timely assignment, resource planning, and instruction evaluation while preserving copy-ready documentation for reporting and institutional memory.

Why a structured Inquiry LOEX Form improves intake and traceability

Who typically completes or receives this form

Use the Inquiry LOEX Form to centralize incoming requests from internal and external stakeholders.

  • Librarians and instructional staff managing course-embedded requests or workshops, ensuring complete contextual details for planning.
  • Faculty and teaching staff requesting instructional support, learning outcomes alignment, or materials creation for classes.
  • Students, researchers, and external partners seeking consultations, data help, or collaboration on resource development.

Central submission ensures consistent routing to the appropriate unit, reduces duplication, and supports transparent response tracking.

Core sections to include in a professional Inquiry LOEX Form

Design the form so each section maps to a decision or action: contact identification, affiliation, request summary, learning goals, scheduling constraints, and deliverables. Clear labels and required fields speed triage.

Contact Details

Collect full name, institutional email, phone, and department to ensure accurate assignment and reliable follow-up for scheduling or clarification.

Affiliation

Record status (faculty, staff, student, external partner), organizational unit, and course identifier when applicable to route to the correct liaison or subject specialist.

Inquiry Type

Specify whether the request is for a workshop, assignment consultation, resource recommendation, or research support so the intake team can prioritize and allocate resources.

Learning Objectives

Ask the requester to state intended learning outcomes or evaluation criteria to align instruction or materials with course goals and assessment plans.

Scheduling & Location

Capture preferred dates, class meeting times, delivery mode (in-person, synchronous online, asynchronous module), and room or platform details to confirm logistics.

Deliverables & Deadlines

Define expected outputs (slides, handouts, reading lists), delivery format, and final due dates to set clear expectations and scope boundaries.

Step-by-step: completing and submitting the Inquiry LOEX Form

Follow these steps to submit a clear, actionable inquiry that can be routed and scheduled without delay.

Where to send or file completed Inquiry LOEX Forms

Decide a single intake destination and define routing rules to avoid parallel handling and lost requests.

  • Central Intake Inbox: Designate an email or ticket queue monitored by intake staff for initial triage.
  • Departmental Routing: Auto-route by inquiry type to subject librarians or instructional teams.
  • Calendar Assignment: Create provisional calendar events for preferred dates pending confirmation.
  • Document Repository: Store submitted materials in a controlled drive with retention policies.

Options for sharing, signing, and storing the form electronically

Choose platforms that support secure submission, simple eSignature, and role-based access controls.

  • Recommended integrations: Google Workspace, Microsoft 365, or an LMS for sending and saving submissions.
  • eSignature support: Use a compliant eSignature provider that supports ESIGN/UETA and audit trails.
  • Access control: SSO, role permissions, and folder-level sharing limit exposure of sensitive records.

Plan to centralize intake, enable audit trails, and apply the institution’s retention rules for submitted records and attachments.

Common mistakes that delay handling of Inquiry LOEX Forms

  • Incomplete contact information forces back-and-forth and slows scheduling by days.
  • Vague request descriptions require additional clarification before work can be scoped effectively.
  • Missing syllabus or assignment prompt prevents alignment with course objectives and lengthens prep time.
  • Unsuitable preferred dates with no alternates cause repeated rescheduling and resource conflicts.

Risks and potential consequences of incorrect or incomplete forms

Delayed response: Task reassignment and workload delays
Misallocated resources: Staffing mismatches and overtime
Privacy exposure: Unsecured attachments risk data breach
FERPA violation: Improper disclosure of student data
Task duplication: Multiple teams working the same request
Reputational harm: Stakeholder frustration and trust erosion

Minimum security and compliance elements to include

Encryption: TLS in transit; AES-256 at rest
Access Control: Role-based permissions and SSO
Audit Trail: Timestamped actions and signer metadata
Privacy Law: Address FERPA or HIPAA applicability
Retention Policy: Document retention and deletion schedule
BAA / Agreements: Execute BAA when PHI is involved

  • Outcome: a focused workshop, tailored handout, and a saved record for later assessment and reuse.

Research Consultation Intake

A graduate student requests a subject-specific literature consultation

  • Intake includes article lists and timeline constraints to scope support quickly
  • Outcome: a scheduled consult, curated resources delivered within SLA, and attached notes stored for program reporting.
