What the Institutional Client Intake Form Is and when it’s used

The Institutional Client Intake Form collects standardized identity, account, and compliance information for organizational clients entering a relationship with a financial institution, advisory firm, custodian, or service provider. It centralizes legal entity details, taxpayer identification (EIN/TIN), authorized signatories, control persons, account types, and any required certifications or attestations needed to open accounts, perform KYC or AML checks, or establish contractual services.

Why standardizing intake matters for institutions

A consistent Institutional Client Intake Form reduces onboarding time, supports regulatory checks, and creates an auditable record for compliance with ESIGN (15 U.S.C. ch. 96), UETA, and industry rules such as HIPAA when handling protected health information.

Why standardizing intake matters for institutions

Who typically completes and reviews this intake form

Different roles interact with the form during onboarding, approval, and recordkeeping.

  • Institutional onboarding teams and compliance officers who verify entity documents and KYC/AML screening results.
  • Authorized signatories and legal representatives who attest to authority and complete signature blocks.
  • Operations staff and account administrators who set up accounts, routing, and integration with back-office systems.

Assign clear internal ownership for each section to avoid gaps during approval and regulatory review.

Representative signers and approvers

Authorized Signer

A corporate officer, partner, or delegated agent with authority to bind the institution. Provide job title, scope of authority, and attach a corporate resolution or power of attorney when required.

Compliance Officer

Security and compliance data points to capture

Encryption: TLS 1.2/1.3 in transit
At-rest: AES-256 encryption
Audit trail: Timestamp and IP logging
Access control: Role-based permissions
HIPAA: BAA when PHI present
Record retention: Tamper-evident storage

Common preparation pitfalls to avoid

  • Submitting inconsistent legal names or abbreviating entity names, which delays verification and may require re-submission.
  • Missing or expired supporting documents such as articles of incorporation, resolutions, or IDs, causing onboarding holds.
  • Omitting taxpayer identification data (EIN/TIN) or providing incorrect TINs that can trigger backup withholding or reporting errors.
  • Using unsecured channels to send sensitive attachments, increasing risk of data breach and violating privacy obligations.
