By submitting this Insurance Binder Request, Applicant requests that the Insurer or its authorized representative issue a temporary binder of coverage subject to the terms herein and subject to the Insurer's underwriting review. This binder, if issued, shall be evidence of temporary insurance coverage only and is not a policy. Coverage shall be binding on the Insurer only upon affirmative written confirmation by the Insurer or its authorized underwriting representative and only to the extent and for the period expressly set forth in that confirmation.

This binder request is subject to the following material conditions: (1) issuance of a formal policy is contingent upon completion of underwriting and receipt of required documentation and premium; (2) coverage under any binder shall be limited to the coverage types, limits, and endorsements expressly accepted in writing by the Insurer; (3) any misrepresentation, omission of material facts, or change in risk prior to issuance of the policy may render this binder void ab initio; and (4) the Insurer reserves all rights to amend, restrict or decline coverage based on underwriting findings.

Binder Expiration: Unless otherwise specified in the Insurer's written confirmation, any binder issued in response to this request will expire upon the earliest of: issuance of the formal policy, the requested termination date above, thirty (30) days from the binder effective date, or receipt by the Insurer of notification of non-acceptance. The Insurer shall not be liable for claims submitted outside the scope or period of the binder.

Applicant represents and warrants that the statements made in this request are true and complete to the best of Applicant's knowledge and that Applicant understands this form's temporary nature. Applicant authorizes the Producer to bind coverage on Applicant's behalf for the limited purpose of obtaining the requested binder and to provide information to the Insurer necessary to evaluate and bind coverage.

Loss Notice / Claims

If coverage is bound, Applicant shall comply with all policy conditions regarding notice of loss and claims. Nothing in this binder request alters any notice, cooperation or claim settlement provisions that will appear in the formal policy.
