By signing this Application, Applicant certifies that all statements contained herein are true, complete and correct to the best of Applicant's knowledge and belief. Applicant understands that any material misrepresentation or omission may give the insurer grounds to void coverage, deny claims, or rescind the policy in accordance with the policy terms and applicable law. Applicant agrees to notify the insurer of any material change in the facts set forth in this Application prior to effective date of the policy.

Applicant authorizes the insurer and its agents to obtain, verify and use motor vehicle records, claims history, medical records, employment records, credit information, and other consumer or underwriting reports as necessary to underwrite the policy, handle claims and to detect and prevent fraud. Applicant further authorizes release of claim and policy information to other insurers, claims databases and regulatory authorities where permitted by law.

Premium, payment schedule, and method of payment shall be determined in the policy and are due as stated in the policy. Failure to pay premium when due may result in policy cancellation in accordance with policy terms. Applicant understands that coverage is not effective until the insurer has accepted this application and any required premium has been paid, except as otherwise specified in a temporary binder issued by the insurer.
