The Insurance Claim AFCTS Form is a standardized claim document used by insurers and claimants to report and document losses, coverage details, and supporting information for adjudication. It collects policy identifiers, claimant contact and payment details, incident descriptions, dates, itemized damages, and witness or police report references. Organizations use AFCTS-formatted claims to streamline intake, ensure consistent evidence capture, and to create an auditable record for underwriting and subrogation. The form supports attachments for photos, estimates, and medical reports and can be completed on paper or as an electronic fillable document.

Why using the AFCTS Form improves claim accuracy and processing

Using the Insurance Claim AFCTS Form standardizes claim intake, reduces information gaps, and speeds adjudication by collecting necessary identifiers and supporting evidence. Consistent entries improve auditability and communication between carriers, adjusters, and third-party vendors while reducing resubmission and processing delays.

  • Insurance carriers and in-house claims teams managing first-notice-of-loss intake and adjudication.
  • Independent adjusters and public adjusters documenting damages, estimates, and evidence for settlement.
  • Policyholders or business owners completing claim details and attaching photos, receipts, or repair estimates.

Primary user roles and responsibilities

Adjuster

Claims adjusters complete the AFCTS Form to consolidate policy and incident data, verify coverage, and document losses. They rely on accurate policy numbers, itemized damage descriptions, and supporting attachments to determine liability, calculate reserves, and communicate decisions to claimants and partners.

Policyholder

Policyholders or insureds use the form to report incidents, list affected property or injuries, and provide authorization for inspections and payment details. Clear, complete entries and timely submission reduce processing delays and correct backup withholding or payment errors.

  • Missing or low-quality attachments—such as unreadable photos, unsigned estimates, or absent medical records—frequently result in denial or requests for additional evidence.
  • Inconsistent dates or mismatched claim amounts between sections trigger audits and can lead to reductions in indemnity or coverage disputes.
  • Failure to obtain required authorizations for inspections or medical releases can prevent verification and may invite regulatory scrutiny.

Fraud Liability: Civil and criminal penalties may apply
Claim Denial: Incomplete or false info can cause denial
