I, the undersigned Applicant/Insured hereby authorize the Insurer named above, and any agent appointed by the Insurer, to draw variable or fixed debit orders against the bank account specified above for payment of premiums, policy fees, levies, and any adjustments arising from endorsements or renewals related to the policy number provided. This authority remains in force until revoked in writing by the Applicant/Insured and acknowledged in writing by the Insurer, subject to any notice period required by applicable banking rules.

The Applicant/Insured accepts responsibility for ensuring sufficient funds are available on the due date. The Applicant/Insured acknowledges that the bank may charge fees for returned or rejected transactions and authorizes the Insurer to recover such fees by separate debit or adjustment to future premiums. The Insurer is authorized to adjust the debit amount to reflect changes to the premium as a result of mid-term adjustments, endorsements, or renewal.

The Applicant/Insured acknowledges that revocation of this mandate does not discharge the obligation to pay any amounts due and does not negate the Insurer's right to effect collection by other lawful means. The Applicant/Insured further agrees that the Insurer may, where required, provide necessary payment instructions and confirmation to the bank to effect this mandate.

Exclusions and Fees

This debit mandate does not cover non-premium amounts unless expressly instructed. Unless agreed in writing, taxes, fines, penalties, or charges unrelated to the insured premium are excluded from the mandate. The Insurer may designate an administrative fee for reinstatement of a policy following failed collection attempts; such fees will be debited in accordance with this mandate where permitted by law.

The form often includes customer disclosure, cancellation instructions, and signature block for handwritten or electronic signature. Insurers use it to reduce manual billing, ensure timely collections, and provide a written record of authorization that supports ACH processing and dispute resolution under applicable banking and consumer protection rules.

Why Insurers and Policyholders Use This Form

Provides clear, bank-authorized consent for recurring premium charges, reduces billing errors, and creates an auditable record for compliance. It standardizes payment terms for insurers and policyholders, supports ACH processing, and helps resolve payment disputes with documented authorization.

Common Preparation Mistakes to Avoid

  • Missing or incorrect bank details lead to ACH returns, processing delays, and extra fees; always verify routing and account numbers against a voided check or bank statement.
  • Failing to obtain clear consent or an explicit notice of electronic transactions can expose the insurer to Reg E disputes and consumer chargebacks.
  • Using insecure transmission methods for bank account numbers increases fraud risk; use encrypted channels and limit emailed account details.
  • Not updating schedules after policy changes causes under- or overcharging; maintain reconciliation processes and notify payers of amount adjustments.

Short-Form Risks and Potential Financial Consequences

Bank Fees: NSF, return, and processing fees.
Consumer Disputes: Unauthorized debit claims under Reg E possible.
Regulatory Risk: Violation of consumer disclosure obligations.
Reputational Harm: Customer complaints and public disputes.
Contract Invalidity: Mismatched signer or missing consent.
Return Limits: ACH return fees and limits apply.
