Payment Authorization

I authorize the insurer named above to initiate electronic debit entries to the bank account indicated above for payment of premiums due under the policy referenced on this form. This authorization remains in effect until revoked in writing in accordance with the terms below. Initial debit instruction: Amount to debit per transaction:

Authorization Terms and Conditions

By signing this form, the Account Holder authorizes the insurer and its service providers to initiate Automated Clearing House (ACH) debits and, if necessary, credit entries and adjustments for any erroneous debits to the bank account specified. Authorization is for the payment of premiums and any applicable fees associated with policy billing. The Account Holder represents that they are either the named account holder or an authorized signer on the account.

The Account Holder acknowledges that the origination of ACH transactions to the account must comply with applicable law and ACH operating rules. The insurer reserves the right to terminate or suspend EFT privileges if a debit is returned unpaid, if there are multiple returned items, or if premium obligations are not satisfied by other means. Returned item fees or other bank fees resulting from insufficient funds or closed accounts are the responsibility of the Account Holder.

To revoke this authorization, the Account Holder must provide written notice to the insurer at least 30 days prior to the requested termination date. Revocation does not affect payments previously initiated in accordance with this authorization or other collection rights of the insurer for unpaid premiums.

The Account Holder agrees to notify the insurer promptly of any change to bank account information. The insurer may rely on the bank account information provided until written notice of change is received and a reasonable opportunity to act on that notice has passed. The Account Holder agrees to indemnify and hold the insurer harmless from any loss, liability or expense arising from inaccurate account information provided by the Account Holder or from failure to timely notify the insurer of a change in account information.

Special Instructions / Additional Authorizations

I certify that I am authorized to execute this EFT Authorization and that all information provided is true and correct. I acknowledge that I have read and understand the terms and conditions contained in this authorization and accept responsibility for all debits initiated pursuant to this authorization.

What the Insurance EFT Authorization Form Is

The Insurance EFT Authorization Form is a written or electronic authorization that permits an insurer, third-party administrator, or payee to debit or credit a policyholder's or payee's bank account via electronic funds transfer (EFT). The form typically captures the account holder's legal name, bank routing number, account number, account type, authorization type (one-time or recurring), effective date, and a signature or electronic consent. When completed electronically in the United States, the form should meet ESIGN and UETA requirements for consent and retention and include any consumer disclosures required for financial or healthcare transactions.

Why a Clear EFT Authorization Matters

A clear Insurance EFT Authorization Form reduces manual check processing and reconciliation effort, documents payer consent for automated transfers, and creates an auditable record to resolve disputes. Properly completed forms help ensure timely premium and claim payments while lowering administrative overhead.

Why a Clear EFT Authorization Matters

Typical Parties Who Complete or Receive This Form

Policyholders, payees, and authorized agents typically initiate or complete the form to establish direct debit or deposit instructions for insurance-related payments.

  • Individual policyholders authorizing recurring premium withdrawals from personal checking or savings accounts for ongoing coverage.
  • Insurance carriers or TPAs collecting bank details to process claim reimbursements and premium adjustments via ACH.
  • Agents and brokers obtaining EFT consent to streamline billing and reduce collection delays for customers.

The completed form is retained by the payer and a copy provided to the signer; electronic versions must meet ESIGN/UETA consent and retention standards to be enforceable.

Essential Sections to Include on a Professional Form

Include account holder details, bank routing and account numbers, authorization terms, processing instructions, effective dates, and clear signature or consent language to create a complete and enforceable authorization.

Account Holder

Provide the legal name of the individual or business that holds the bank account. Names must match the bank's records to avoid returned transactions and delayed processing.

Routing Number

Enter the nine-digit ABA routing transit number exactly as shown on checks or bank statements. An incorrect routing number will cause ACH rejection and may incur bank fees or payment delays.

Account Number

Record the full account number without spaces or dashes. Confirm the number with a bank document or voided check; mismatches commonly lead to returned payments and manual follow-up.

Authorization Type

Specify whether the authorization is one-time, recurring, or conditional. Include maximum amounts, frequency, and any limits; clear terms reduce disputes and support automated processing.

Effective Date

Enter the date when transfers may begin in MM/DD/YYYY format. The effective date determines the first authorized debit or credit and affects cancellation timing and notice periods.

Signature

Collect a handwritten or electronic signature and the signing date. When signed electronically, include consent language per ESIGN and capture an audit trail showing signer identity and timestamp.
