Policy Provisions and Conditions

1. Payment of Benefits: Subject to the terms, conditions and exclusions of this Policy, the Insurer will pay the Death Benefit to the designated beneficiary upon receipt by the Insurer of due proof of death and any required documentation.

2. Contestability: Statements in the Application for this Policy are representations and not warranties. Except for non-payment of premiums, the validity of this Policy shall not be contested after two (2) years from the Issue Date based on statements in the Application, unless fraud is involved.

3. Suicide: If the Insured dies by suicide within two (2) years from the Issue Date, the Insurer's liability shall be limited to a refund of premiums paid, less any indebtedness, unless otherwise required by law.

4. Grace Period and Lapse: A grace period of thirty-one (31) days is provided for payment of any premium due. If any premium remains unpaid at the end of the grace period, the Policy shall lapse subject to any available non-forfeiture benefit.

5. Assignment: This Policy may be assigned in whole or in part. The Insurer shall not be bound by any assignment until it has received written notice and has recorded such assignment at its Home Office. The Insurer assumes no responsibility for the validity or sufficiency of any assignment.

6. Loans and Cash Values: If applicable under the Policy form selected, Policy loans and cash values are subject to the terms, interest rates and conditions set forth in the Policy schedule and applicable law.

7. Currency: All amounts payable under this Policy shall be payable in lawful currency of the United States unless otherwise agreed in writing.

Claims and Notices

Upon the Insured's death, the beneficiary must provide written notice, certified death certificate, proof of relationship if requested, and any other documentation reasonably required by the Insurer to process the claim. The Insurer shall pay claims within a reasonable period after receipt of due proof of loss.

Declarations and Applicant Certification

The Applicant declares that the statements and answers contained in the Application and this Policy are complete and true to the best of the Applicant's knowledge and belief. These statements are the basis for the Policy and will be considered as representations made to the Insurer. The Applicant agrees that the Policy shall be issued subject to the terms, conditions and exclusions stated herein.
